摘要
目的探讨类风湿性关节炎(RA)抗核抗体(ANA)和类风湿因子(RF)双阳性与双阴性患者的临床特征。方法将2018年1月至2021年12月在本院接受诊治的309例RA患者为研究组,同时抽取50例在本院体检中心接受正规体检的健康志愿者为对照组。分析两组一般资料、临床资料及实验室指标。结果研究组的RF阳性、ANA阳性、ANA与RF双阳性率高于对照组,ANA与RF双阴性率低于对照组(P<0.05)。ANA与RF双阳性患者的关节肿胀数、视觉模拟评分法(VAS)、28个关节疾病活动度(DAS28)评分及骨质侵蚀、贫血、干燥综合征发生率显著高于ANA与RF双阴性患者(P<0.05)。ANA与RF双阳性患者的抗环瓜氨酸化多肽抗体(ACPA)阳性率和C反应蛋白(CRP)、血沉(ESR)、免疫球蛋白G(IgG)水平均显著高于ANA与RF双阴性患者(P<0.05)。结论与ANA、RF双阴性患者相比,ANA、RF双阳性RA患者的疼痛更明显,疾病活动度更高,贫血和干燥综合征发生率更高,免疫炎症反应更强烈。
Objective To explore the clinical characteristics between antinuclear antibody(ANA)and rheumatoid factor(RF)double positive and double negative patients with rheumatoid arthritis(RA).Methods A total of 309 RA patients who were diagnosed and treated in our hospital from January 2018 to December 2021 were selected as study group,and 50 healthy volunteers who received regular physical examination in the physical examination center of our hospital were selected as control group.General data,clinical data and laboratory indexes of both groups were analyzed.Results The RF positive,ANA positive,ANA and RF double positive rates of the study group were higher than those of the control group,and the ANA and RF double negative rate was lower than that of the control group(P<0.05).The number of joint swelling,Visual Analogue Scale(VAS),Disease Activity Score in 28 Joints(DAS28)scores and the incidence of bone erosion,anemia and Sjogren's syndrome in ANA and RF double positive patients were significantly higher than those in ANA and RF double negative patients(P<0.05).The positive rate of anti-cyclic citrullinated polypeptide antibodies(ACPA)and the levels of C reactive protein(CRP),erythrocyte sedimentation rate(ESR)and immunoglobulin G(IgG)in ANA and RF double positive patients were significantly higher than those in ANA and RF double negative patients(P<0.05).Conclusion Compared with patients with ANA and RF double negative,RA patients with ANA and RF double positive has more obvious pain,higher disease activity,higher incidences of anemia and Sjogren's syndrome,and stronger immune inflammatory response.
