摘要
目的:探讨基于NRS 2002评分的营养管理模式对脑梗死吞咽障碍患者营养状况的影响。方法:选取2022年6月—2023年6月苏州高新区人民医院神经内科收治的92例脑梗死吞咽障碍患者为研究对象。按照随机数字表,将92例患者平均分为对照组和观察组各46例。对照组患者接受常规营养管理,给予观察组患者基于NRS 2002评分的综合性营养管理。观察两组患者干预1月后血清营养指标白蛋白(ALB)、前白蛋白(PA)、血红蛋白(HGB),吞咽功能(采用洼田饮水试验评估)及肺部感染、电解质紊乱、恶心/呕吐、便秘/腹泻等并发症的变化情况。结果:经营养管理后,2组患者血清营养指标均较前改善,而基于NRS 2002指导下的营养管理模式较常规营养管理模式对患者血清ALB[(35.48±1.80)vs(33.27±1.98)g/L)、PA[(215.83±22.35)vs(194.72±21.57)mg/L]及HGB[(110.56±7.21)vs(104.61±6.68)g/L]增高水平更高,均有P<0.05。观察组吞咽功能改善有效率为60.87%,显著高于对照组(34.78%,χ^(2)=6.273,P<0.05);而并发症发生率显著低于对照组(13.04%vs 34.78%,χ^(2)=5.974,P<0.05)。结论:基于NRS 2002评分的营养管理模式可显著改善脑梗死吞咽障碍患者营养状况、吞咽功能,降低患者并发症发生率。
【Objective】To explore the effect of a nutritional management model based on NRS 2002 on the nutritional status of patients with cerebral infarction with dysphagia.【Method】Totally 92 patients with cerebral infarction dysphagia admitted to the Department of Neurology of Suzhou High-tech Zone People's Hospital from June 2022 to June 2023 were selected for the study.According to the random number table,the 92 patients were evenly divided into 46 cases each in the control group and the observation group.The patients in the control group received conventional nutritional management,and the patients in the observation group were given comprehensive nutritional management based on NRS 2002.Changes in serum nutritional indicators albumin(ALB),prealbumin(PA),haemoglobin(HGB),swallowing function and complications such as pulmonary infections,electrolyte disorders,nausea/vomiting,and constipation/diarrhoea were observed in the patients in the two groups after 1 month of intervention【.Result】After nutritional management,the serum nutritional indexes of patients in both groups were improved compared with the previous ones,while the nutritional management mode based on the guidance of NRS 2002 was more effective than the conventional nutritional management mode in increasing the level of patients'serum ALB,PA,and HGB increased at a higher leve(l all with P<0.05).The effective rate of improvement of swallowing function in the observation group was 60.87%,which was significantly higher than that in the control group.The complication rate was significantly lower than that in the control group.【Conclusion】Nutritional management model based on NRS 2002 can significantly improve the nutritional status and swallowing function of patients with cerebral infarction dysphagia and reduce the complication rate of patients.
