摘要
目的:分析经腹、经阴道超声检查在子宫腺肌瘤中的诊断效能.方法:选取2022年3月至2023年3月本院收治的88例疑似子宫腺肌瘤的患者作为研究对象,将其纳入疑似子宫腺肌瘤组.另选取同期本院收治的80例子宫肌瘤患者,纳入子宫肌瘤组.所有患者均采用经腹部超声检查和经阴道超声检查.以病理学检查结果作为诊断的"金标准",分析经腹、经阴道超声检查在子宫腺肌瘤诊断及鉴别诊断中的诊断效能.结果:疑似子宫腺肌瘤组患者经病理学诊断后,77例证实为子宫腺肌瘤.经阴道超声检查诊断子宫腺肌瘤的灵敏度、特异度、准确度、阳性预测值及阴性预测值均高于经腹部超声检查,差异均有统计学意义(P<0.05).子宫腺肌瘤患者病灶周围血流阻力指数(RI)及波动指数(PI)均显著大于子宫肌瘤患者,差异均有统计学意义(P<0.05).结论:经阴道超声检查应用于子宫腺肌瘤中的诊断效能更高,且病灶周围RI、PI值有助于与子宫肌瘤进行鉴别诊断.
Objective:To analyze the diagnostic efficacy of transabdominal and transvaginal ultrasonography in adenomyoma.Methods:Eighty-eight patients with suspected adenomyoma admitted to our hospital from March 2022 to March 2023 were selected in this study.They were included in the suspected adenomyoma group.Additionally,80 patients with uterine fibroids during the same period were selected as the uterine fibroid group.All patients underwent transabdominal ultrasonography and transvaginal ultrasonography,respectively.Using pathological examination results as the"gold standard"for diagnosis,the diagnostic efficacy of transabdominal and transvaginal ultrasound in the diagnosis and differential diagnosis of adenomyoma were analyze.Results:After pathological diagnosis,77 patients with suspected adenomyoma were confirmed to have adenomyoma.The sensitivity,specificity,accuracy positive predictive value and negative predictive value of transvaginal ultrasonography in diagnosing adenomyoma were higher than those of transabdominal ultrasonography,the differences were statistically significant(P<0.05).The resistance index(RI)and pulsatility index(PI)values around the lesion in patients with adenomyoma were significantly higher than those in patients with uterine fibroids,the differences were statistically significant(P<0.05).Conclusion:The diagnostic efficiency of transvaginal ultrasonography in adenomyoma is higher than that of transabdominal ultrasonography.The RI and PI values around the lesion are conducive to differential diagnosis with uterine fibroids.
