摘要
金元时期医家李东垣提出“脾胃气虚,阴火邸张”,并以此为基础形成“阴火”理论,气虚与“阴火”关系密切,脾胃气虚在“阴火”的形成中发挥重要作用。糖尿病肾脏病是糖尿病常见的慢性并发症,炎症反应是糖尿病肾脏病发生的主要机制之一,气虚也是糖尿病肾脏病炎症产生的重要基础,而脾胃运化失常在糖尿病患者肾脏发生炎症反应的过程中发挥了重要作用,糖尿病肾脏病炎症机制与“阴火”理论相契合。当代医家也有依据“阴火”理论来论治糖尿病肾脏病者,取得较好的临床疗效。该研究拟从东垣学说、炎症机制学说、当代医家对糖尿病肾脏病认识三个角度对“阴火”理论与糖尿病肾脏病炎症机制的关系作初步论述,旨在进一步完善糖尿病肾脏病的中医理论,从而为糖尿病肾脏病临床诊断和治疗提供理论依据。
LI Dongyuan,a medical doctor in the Jin-Yuan period,proposed the theory of“Yinfire”based on“Qi deficiency in the spleen and stomach and Yin-fire diaphoresis”.Qi deficiency is closely related to“Yin-fire”,and Qi deficiency in the spleen and stomach plays an important role in the formation of“Yin-fire”.Diabetic kidney disease is a common chronic complication of diabetes mellitus,and inflammation is one of the main mechanisms of diabetic kidney disease,and Qi deficiency is also an important basis for inflammation in diabetic kidney disease.The inflammatory mechanism of diabetic kidney disease is consistent with the theory of“Yin-fire”.The inflammatory mechanism of diabetic kidney disease is consistent with the“Yin-fire”theory.Contemporary medical practitioners have also treated diabetic kidney disease based on the“Yin-fire”theory and achieved good clinical results.In this study,we intend to make a preliminary discussion on the relationship between the theory of“Yin-fire”and the inflammatory mechanism of diabetic kidney disease from three perspectives:the Dongyuan theory,the theory of inflammatory mechanism,and the understanding of diabetic kidney disease by contemporary physicians,with the aim of further improving the TCM theory of diabetic kidney disease and providing a theoretical basis for the clinical diagnosis and treatment of diabetic kidney disease.
