摘要
目的:探究Masimo SedLine监测不同麻醉深度指导地佐辛复合丙泊酚在妊娠终止术中的应用及血清COR、E水平的差异性。方法:本研究以我院进行终止妊娠的患者186例作为研究对象,按照简单随机原则,分为观察组和对照组各93例。对照组采取地佐辛复合丙泊酚麻醉治疗,观察组患者采取Masimo SedLine监测不同麻醉深度辅以地佐辛复合丙泊酚麻醉治疗。比较两组患者的血流动力学指标、麻醉指标、疼痛情况、血清COR、E水平以及不良反应的差异。结果:相比对照组,观察组患者的收缩压、舒张压、血氧饱和度以及心率在不同的时点波动情况显著低于对照组(均P<0.05),两组患者的手术时间差异无统计学意义(P>0.05),观察组患者的丙泊酚使用量(t=6.737,P=0.000)、诱导时间(t=8.384,P=0.000)以及苏醒时间(t=17.090,P=0.000)均小于对照组,两组患者的不同时间的VAS评分显著低于对照组,T0阶段,两组患者的血清COR、E水平之间的差异无统计学意义(P>0.05),T6阶段,两组患者的血清COR、E水平均显著升高,且观察组患者的血清COR(t=19.868,P=0.000)、E(t=27.134,P=0.000)水平显著低于对照组,观察组患者发生术中体动(χ^(2)=4.731,P=0.030)、呼吸抑制(χ^(2)=5.142,P=0.023)、术后宫缩痛(χ^(2)=5.721,P=0.017)情况低于对照组。结论:在妊娠终止术中,Masimo SedLine监测不同麻醉深度能指导地佐辛复合丙泊酚麻醉剂量,对患者的麻醉作用以及血清氧化应激反应的降低均有显著优势。
Objective:To study the application of Masimo SedLine monitoring-guided use of dexmedetomidine combined with propofol in termination of pregnancy and the difference in serum cortisol(COR)and epinephrine(E)levels.Methods:In this study,186 patients who underwent termination of pregnancy in our hospital were selected as the research objects.According to the principle of random grouping,these patients were divided into an observation group and a control group,with 93 in each.The control group received dexmedetomidine combined with propofol anesthesia,while the observation group received Masimo SedLine monitoring-guided use of dexmedetomidine combined with propofol anesthesia.The differences in hemodynamic indicators,anesthesia indicators,pain status,serum COR and E levels,and adverse reactions between the two groups were compared.Results:Compared with the control group,the fluctuation of systolic blood pressure,diastolic blood pressure,blood oxygen saturation and heart rate at different time points in the observation group was significantly lower than that in the control group(P<0.05).There was no statistical significance between the two groups regarding operation time(P>0.05).The amount of propofol used(t=6.737,P=0.000),induction time(t=8.384,P=0.000)and recovery time(t=17.090,P=0.000)in the observation group were lower than those in the control group;VAS scores at different times were significantly lower than those in the control group;there was no significant difference between serum COR and E levels between two groups at T0 stage(P>0.05),but at T6 stage,the serum COR and E levels in both groups were significantly increased,and the serum COR(t=19.868,P=0.000)and E(t=27.134,P=0.000)levels in the observation group were significantly lower than those in the control group;the incidence of intraoperative body movement(χ^(2)=4.731,P=0.030),respiratory depression(χ^(2)=5.142,P=0.023)and postoperative uterine contraction pain(χ^(2)=5.721,P=0.017)in the observation group were lower than those in the control group.Conclus
