摘要
目的探究食管早癌患者内镜黏膜下剥离术后疾病不确定感潜在类别及其特征差异。方法选取2022年6月至2023年6月在南京大学医学院附属鼓楼医院接受内镜黏膜下剥离术治疗的184例食管早癌患者作为研究对象,采用一般资料调查表、疾病不确定感量表、医学应对方式量表、简易疾病感知问卷进行调查。结果食管早癌患者疾病不确定感可分为低水平不确定感组(26.1%)、中等水平不确定感组(45.8%)、高水平不确定感组(28.1%)3种类别。Logistic回归分析显示,年龄、疾病分期、食管狭窄、疼痛评分、医学应对方式是疾病不确定感的影响因素(P<0.05)。结论食管早癌患者的疾病不确定感可分为3个潜在剖面,建议医护人员依据分类特征采取针对性的干预策略,帮助患者以积极心态应对疾病,降低疾病不确定感,在一定程度上减少复发风险。
Objective To explore the potential categories of illness uncertainty of early esophageal cancer patients.Methods 184 patients with early esophageal cancer who received Endoscopic Submucosal Dissection treatment in a tertiary-level hospital in Nanjing from June 2022 to June 2023 were selected as the study subjects,and were investigated The general information questionnaire,Mishel Uncertainty in Illness Scale,Medical Coping Modes Questionnaire,Brief Illness Perception Questionnaire.Results Illness uncertainty in early esophageal cancer patients could be divided into three potential categories:low level uncertainty group(26.1%),medium level uncertainty group(45.1%),and high level uncertainty group(28.8%).Logistic regression analysis showed that different age,disease staging,oesophageal stenosis,VAS scores,medical coping modes were the influencing factors of patients′uncertainty in illness(P<0.05).Conclusion Illness uncertainty of early esophageal cancer patients can be divided into 3 categories,and healthcare professionals should formulate targeted intervention strategies according to the characteristics,so as to enable patients to cope with the disease with a positive mindset,reduce the sense of uncertainty about the disease,and reduce the risk of recurrence to a certain extent.
