摘要
目的探究超声引导下腹横肌平面阻滞用于腹腔镜全子宫切除术后镇痛效果分析。方法选取2020年3月—2022年5月本院接受腹腔镜全子宫切除术患者90例为研究对象,以入院手术时间完成分组,对照组、观察组各纳入45例,对照组接受静脉自控镇痛(PCIA),观察组在对照组基础上加施超声引导下腹横肌平面阻滞(TAPB),比较两组镇痛效果。结果术后2h、术后24h观察组VAS评分较对照组低,BCS评分较对照组高(P<0.05);观察组术后2h、术后12h、术后24h时间段MAP、心率血流动力学指标水平均较对照组低(P<0.05);术后2h、术后24h观察组ACTH、CRH、Cor应激反应指标水平均较对照组低(P<0.05);观察组不良反应与对照组比较无明显差异(P>0.05)。结论超声引导下腹横肌平面阻滞可提高镇痛效果,调节血流动力学,缓解机体应激反应,且具有较高安全性,值得推广。
Objective To explore analgesic effect of ultrasound-guided transverse abdominis plane block for laparoscopic total hysterectomy.Methods The paper chose 90 patients with laparoscopic total hysterectomy in our hospital from March 2020 to May 2022 as study subjects,and divided them into two groups based on admission,with 45 cases in control group and 45 cases in observation group.Control group was treated with patient-controlled intravenous analgesia(PCIA),while observation group with ultrasound-guided transverse abdominis plane block(TAPB)based on control group.Analgesic effect between two groups was compared.Results VAS scores of observation group were lower than control group at 2 hours and 24 hours after surgery,while BCS scores were higher than control group(P<0.05).Levels of MAP,heart rate,and hemodynamic indicators in observation group were lower than control group at 2 hours,12 hours,and 24 hours after surgery(P<0.05).Levels of ACTH,CRH,and Cor stress response indicators in observation group were lower than control group at 2 hours and 24 hours after surgery(P<0.05).There was no significant difference in adverse reactions between observation group and control group(P>0.05).Conclusion Ultrasound guided transverse abdominis plane block can improve analgesic effect,regulate hemodynamics,alleviate stress response in the body with high safety,which is worthy of promotion.
