摘要
目的:探讨BiPAP无创呼吸机配合常规治疗对慢性肺源性心脏病(CPHD)患者血清氨基末端B型脑钠肽前体(NT-proBNP)、肺功能及血气分析指标的影响。方法:选择2020年1月—2022年12月本院60例CPHD患者,随机数字表法分为对照组(30例)和观察组(30例),对照组实施常规治疗,观察组在对照组基础上采用BiPAP无创呼吸机配合治疗。治疗前、治疗7 d后测定两组血清NT-proBNP水平、肺功能指标[一秒用力呼气容积(FEV1)、用力肺活量(FVC)、并比较两者比值即FEV1/FVC]、血气分析指标[动脉二氧化碳分压(PaCO_(2))、动脉氧分压(PaO_(2))和pH]并比较。结果:治疗前两组NT-proBNP、FVC、FEV1、FEV1/FVC、PaCO_(2)、PaO_(2)和pH比较,差异无统计学意义(P>0.05);治疗7 d后,两组血清NT-proBNP水平和PaCO_(2)均降低,且观察组低于对照组(P<0.05);两组FVC、FEV1、FEV1/FVC、PaO_(2)和pH均升高,且观察组高于对照组,差异有统计学意义(P<0.05)。结论:BiPAP无创呼吸机配合常规治疗对CPHD患者的NT-proBNP水平、肺功能和血气分析指标具有显著的改善作用。
Objective:To explore the effects of BiPAP noninvasive ventilator combined with routine treatment on serum N-terminal B-type brain natriuretic peptide precursor(NT proBNP),lung function,and blood gas analysis indicators in patients with chronic pulmonary heart disease(CPHD).Methods:Sixty patients with CPHD from the Third Hospital of Pingxiang City from January 2020 to December 2022 were selected and randomly divided into a control group(30 cases)and an observation group(30 cases)using a number table method.The control group received routine treatment,while the observation group received BiPAP noninvasive ventilation combined with treatment.Before and seven days after treatment,serum NT proBNP levels,lung function indicators(forced expiratory volume in one second(FEV1),forced vital capacity(FVC),and their ratio(FEV1/FVC),blood gas analysis indicators(arterial partial pressure of carbon dioxide(PaCO_(2)),arterial partial pressure of oxygen(PaO_(2)),and pH)were measured and compared between the two groups.Before treatment,there was no statistically significant difference in the NT-proBNP,FVC,FEV1,FEV1/FVC,PaCO_(2),PaO_(2),and pH levels between the two groups(P>0.05).After seven days of treatment,the serum levels of NT-proBNP and PaCO_(2)in both groups decreased,and the observation group was lower than the control group.FVC,FEV1,FEV1/FVC,PaO_(2),and pH in both groups increased,and the observation group was higher than the control group,with a statistically significant difference(P<0.05).Conclusions:BiPAP noninvasive ventilator combined with routine treatment significantly improves the NT proBNP levels,lung function,and blood gas analysis indicators in patients with CPHD.
