摘要
目的 分析四川省苯作业人员职业健康检查血常规异常情况和影响因素,为预防和控制苯所致的职业危害提供依据。方法 收集四川省职业病防治综合管理信息系统和中国疾病预防控制中心职业病与健康危害因素监测信息系统中2020—2022年四川省苯作业工人全部在岗和离岗职业健康检查数据,用SPSS 26.0软件对血常规检查结果以及影响因素进行分析,组间比较采用Pearson χ~2检验,影响因素分析采用二元Logistic回归,检验水准α=0.05。结果 81 420位苯作业人员中有4 399人血常规异常,检出率5.45%,女性检出率(7.36%)高于男性(4.37%),差异有统计学意义(χ~2=326.494,P<0.001),随着年龄增加血常规异常率呈逐渐升高趋势(χ~2_(趋势)=77.461,P<0.001);多因素Logistic回归分析结果显示,性别女、年龄较大、工龄较短和企业规模(小微型)是苯作业人员血常规异常的危险因素(P均<0.05)。结论 四川省苯暴露基数较大,小微型私有企业的女性苯作业人员是重点人群,应加强职业防护和职业卫生知识培训,最大限度保护其职业健康。
Objective To analyze the blood routine in occupational health examination of workers exposed to benzene in Sichuan Province and its influencing factors,and to provide basis for prevention and control of occupational hazard caused by benzene.Methods Occupational health examination data of all on-job and off-job workers exposed to benzene in Sichuan Province from 2020 to 2022 were collected from the Comprehensive Management Information System for Occupational Disease Prevention and Control in Sichuan Province and the Occupational Disease and Health Hazard Factor Monitoring Information System of Chinese Center for Disease Control and Prevention.SPSS 26.0 software was used to analyze the results of blood routine examination and influencing factors.Pearsonχ~2test was used for inter-group comparison.Binary Logistic regression was used for influencing factor analysis,and the test level was α=0.05.Results Among 81 420 benzene exposed workers,4 399 had abnormal blood routine,with a detection rate of 5.45%.The detection rate in women (7.36%) was higher than that in men (4.37%),and the difference was statistically significant(χ~2=326.494,P<0.001).The abnormal blood routine rate gradually increased with the increase of age(χ~2_(trend)=77.461,P<0.001).The results of multivariate Logistic regression analysis showed that female,older age,shorter working experience and small or micro enterprise scale were the risk factors for abnormal blood routine of benzene exposed workers (all P<0.05).Conclusions The benzene exposure base in Sichuan Province is relatively large,and female benzene exposed workers in small and micro private enterprises are the key groups.Occupational protection and occupational health knowledge training should be strengthened to maximize the protection of their occupational health.
