2022年广安市12岁儿童口腔健康状况及龋患率影响因素分析 被引量:1

Oral health status and influencing factors among 12-year-old children in Guang'an City in 2022

摘要 目的了解广安市12岁儿童牙齿患龋现状及其影响因素,为广安市儿童口腔保健工作提供基础资料和科学依据。方法2022年在广安市采用分层整群随机抽样方法,选择两个城市和两个农村县区,对其中15所学校的1054名在校学生进行口腔健康相关内容的问卷调查和口腔体格检查,儿童口腔龋患率影响因素的单因素分析用χ~2检验,多因素分析用二元Logostic回归分析,除特别说明外,检验水准α=0.05。结果2022年共调查广安市12岁儿童1054人,其中有465名儿童患有龋病,患龋率为44.12%,龋均为1.15,其中城市儿童患龋率为51.27%(364/710),高于农村儿童29.36%(101/344),差异有统计学意义(χ~2=45.11,P<0.01);城市儿童龋均为1.41,高于农村儿童0.59,差异有统计学意义(t=-7.44,P<0.01);有138名儿童牙龈出血,牙龈出血检出率为13.09%(138/1054),223名儿童有牙石,牙石检出率为21.11%(223/1054),城市儿童牙龈出血检出率(χ~2=16.80,P<0.01)和牙石检出率(χ~2=56.62,P<0.01)略高于农村儿童,差异有统计学意义。Logistic回归分析结果显示,父母是否长期与孩子同住(OR=2.48,95%CI:1.32~4.68,P=0.01)、牙龈是否出血(OR=6.82,95%CI:2.73~17.03,P<0.01)、牙齿发育情况(OR=12.58,95%CI:3.42~46.19,P<0.01)是农村儿童是否患龋的影响因素;吃零食的频率(OR=1.98,95%CI:1.25~3.13,P=0.01)、是否住校(OR=0.62,95%CI:0.39~0.98,P=0.04)是城市儿童是否患龋的影响因素。结论广安市12岁儿童患龋率整体较高,牙龈出血检出率和牙石检出率较低,相较于农村儿童,城市儿童口腔状况更加严峻,应针对城乡儿童不同的影响因素,加强儿童口腔疾病预防防治工作。 Objective To investigate the dental caries status and influencing factors of 12-year-old children in Guang'an City,and to provide basic data and scientific basis for children's oral health care in Guang'an City.Methods In 2022,two cities and two rural areas were selected by using the stratified cluster sampling method in Guang'an City,and 1054 students in 15 schools were surveyed and orally examined.Chi-square test was used for univariate analysis of the influencing factors of oral caries rate in children,and binary Logostic regression was used for multivariate analysis,and the significance level was set to 0.05 unless α otherwise specified.Results In 2022,a total of 1054 children aged 12 years in Guang'an were surveyed.Overall,465 children had caries,the caries rate was 44.1%,and caries average was 1.15.The prevalence of caries in urban children was 51.27%(364/710),which was higher than that in rural children(29.36%,101/344),with statistically significant difference (χ~2=45.11,P<0.01).The mean caries of urban children was 1.41 higher than that of rural children(1.41 vs.0.59,t=-7.44,P<0.01).Overall,138children had gum bleeding,the detection rate of gingival bleeding was 13%(138/1 054),223children had tartar,the detection rate of tartar was 21.11%(223/1 054),the detection rate of gingival bleeding (χ~2=16.80,P<0.01) and calculus (χ~2=56.62,P<0.01) in urban children was slightly higher than that in rural children.Logistic regression analysis showed parents lived with their children for a long time(OR=2.48,95%CI:1.32-4.68,P=0.01),bleeding gums(OR=6.82,95%CI:2.73-17.03,P<0.01),and dental development(OR=12.58,95%CI:3.42-46.19,P<0.01)were the influencing factors of caries in rural children,and the frequency of snacking(OR=1.98,95%CI:1.25-3.13,P=0.01),and lived in school(OR=0.62,95%CI:0.39-0.98,P=0.04) were the influencing factors of caries in urban children.Conclusions The overall caries rate of 12-yearold children is higher in Guang'an City.The detection rate of gum bleeding and tartar is low.Compared with rural c
