摘要
目的探讨单孔腹腔镜治疗肠套叠中腹内复位与腹外复位的疗效。方法将新乡医学院第一附属医院2018年10月至2021年11月以彩色多普勒超声仪确诊的81例肠套叠患儿作为研究对象,所有患儿均行单孔腹腔镜治疗,治疗过程中根据复位方法不同分为腹内复位组和腹外复位组。收集患儿临床指标:血便、阵发性哭闹或腹痛、呕吐、腹部包块、肠套叠类型、病程;记录患儿手术时间、术中出血量、住院时间、术后排气/排便时间;记录术后感染、出血、肠黏连、肠梗阻、肠穿孔、复发情况和患儿家属满意度情况。结果腹内复位组、腹外复位组临床特征、肠套叠类型、病程、术中出血量、术后排气/排便时间、住院时间、术后感染、出血、肠黏连、肠梗阻、肠穿孔情况差异均无统计学意义(P>0.05),两组患儿均未复发肠套叠;与腹内复位组相比,腹外复位组手术时间降低、患儿家属满意度增加(P<0.05)。结论单孔腹腔镜治疗肠套叠中腹外复位患儿手术时间短、患儿家属满意度高,值得临床推广。
Objective To explore the efficacy of intra-abdominal reduction and extra-abdominal reduction in single port laparoscopy assisted treatment for intussusception.Methods A total of 81 children with intussusception diagnosed by color doppler ultrasound at the First Affiliated Hospital of Xinxiang Medical University from October 2018 to November 2021 were selected as the study subjects.They all underwent single port laparoscopy assisted treatment.And they were divided into intra-abdominal reduction group and extra-abdominal reduction group based on different reduction methods during the treatment.Clinical indicators were collected:bloody stools,paroxysmal crying or abdominal pain,vomiting,abdominal masses,type of intussusception,and course of disease.Duration of operation,intraoperative bleeding,hospital stay,and postoperative exhaust/defecation time were recorded.And postoperative indicators were also recorded:postoperative infection,bleeding,intestinal adhesion,intestinal obstruction,intestinal perforation,recurrence and satisfaction of family members of the children.Results There were no significant differences in clinical characteristics,type and course of intussusception,volume of intraoperative bleeding,postoperative exhaust/defecation time,hospital stay,postoperative infection,bleeding,intestinal adhesion,intestinal obstruction,and intestinal perforation between the two groups(P>0.05).There was no recurrence of intussusception in both groups.Compared with the intra-abdominal reduction group,the extra-abdominal reduction group had decreased duration of operation and increased satisfaction of family members of the children(P<0.05).Conclusion Extra-abdominal reduction in single port laparoscopy assisted treatment for intussusception in children shortens the duration of operation and increases the satisfaction of family members,which is worthy of clinical promotion.
