摘要
目的探讨N端⁃脑钠肽前体(N⁃terminal pro⁃brain natriuretic peptide,NT⁃pro BNP)、内皮素(endothelin,ET)、尾加压素Ⅱ(urotensinⅡ,UⅡ)在妊娠期高血压Ⅰ~Ⅱ期眼底病变预测及病情评估中的应用效果。方法选取2019年2月至2022年2月北京市垂杨柳医院198例妊娠期高血压患者作为观察组,有眼底病变110例,无眼底病变88例,另纳入86例正常孕妇作为对照组。比较两组血清NT⁃pro BNP、ET、UⅡ表达、最佳矫正视力(best corrected visual acuity,BCVA)、视敏度,Spearman相关系数分析各指标间关系,绘制受试者工作特征曲线(receiver operating characteristic curve,ROC)及曲线下面积分析血清各指标预测效能。结果观察组有眼底病变患者血清NT⁃pro BNP、ET、UⅡ水平高于无眼底病变患者及对照组,且无眼底病变患者高于对照组(P<0.05);Ⅰ期眼底病变患者血清NT⁃pro BNP、ET、UⅡ水平低于Ⅱ期眼底病变患者,BCVA、视敏度高于Ⅱ期眼底病变(P<0.05);Spearman相关系数显示,妊娠期高血压患者眼底病变程度与NT⁃pro BNP、ET、UⅡ呈正相关,与BCVA、视敏度呈负相关,且BCVA、视敏度与NT⁃pro BNP、ET、UⅡ均呈负相关(P<0.05);ROC曲线显示,三者联合预测妊娠期高血压眼底病变价值优于单一预测。结论妊娠期高血压眼底病变患者血清NT⁃pro BNP、ET、UⅡ水平与病情程度、BCVA、视敏度相关,联合检测有助于提高妊娠期高血压眼底病变预测价值。
Objective To investigate the effect of N⁃terminal pro⁃brain natriuretic peptide(NT⁃pro BNP),endothelin(ET)and urotensinⅡ(UⅡ)in the prediction and assessment of fundus lesions in gestational hypertension stage I~II.Methods 198 patients with gestational hypertension in Beijing Liuyang Hospital from February 2019 to February 2022 were selected as the observation group,110 with fundus lesions,88 without fundus lesions,and 86 normal pregnant women were included as the control group.The expression of NT⁃pro BNP、ET、UⅡin serum,best corrected visual acuity(BCVA)and visual acuity of the two groups were compared,and the Spearman correlation coefficient was used to analyze the relationship between the indexes,receiver operating characteristic(ROC)curve and area under the curve were drawn to analyze the predictive efficacy of serum indexes.Results The serum levels of NT⁃pro BNP,ET and UII in the patients with fundus lesions in the observation group were higher than those in the patients without fundus lesions and the control group,and the patients without fundus lesions were higher than those in the control group(P<0.05).The serum levels of NT⁃pro BNP,ET and UⅡin patients with stage I fundus lesions were lower than those in patients with stage II fundus lesions,while BCVA and visual acuity were higher than those in patients with stage II fundus lesions(P<0.05).Spearman correlation coefficient showed that the degree of fundus lesions in patients with gestational hypertension was positively correlated with NT⁃pro BNP,ET,and UII,and negatively correlated with BCVA and visual acuity,and BCVA and visual acuity were negatively correlated with NT⁃pro BNP,ET and UII(P<0.05).The ROC curve showed that the combination of the three was better than the single prediction in predicting the fundus lesions of gestational hypertension.Conclusion The serum levels of NT⁃pro BNP,ET and UII in patients with gestational hypertension fundus lesions are related to the disease severity,BCVA and visual acuity.Combined dete
