克罗恩病患者杏仁核亚区脑功能改变的fMRI研究

摘要 目的:探索克罗恩病(CD)患者杏仁核亚区的自发活动水平和局部一致性的改变。方法:30名CD患者和30名年龄、性别和教育程度相匹配的健康对照组接受了静息态功能磁共振成像,并完成了医院焦虑和抑郁量表(HADS)测试。在Matlab软件中使用REST工具包计算被试杏仁核各亚区(ALFF)值以及Reho值。在SPSS 20.0软件中使用双样本t检验比较两组被试的杏仁核3个亚区(基底外侧核、中央内侧核、浅表核)的低频振荡振幅(ALFF)值以及局部一致性(Reho)值。结果:在HADS⁃A评分及HADS⁃D评分中,CD患者评分显著高于健康对照组(P<0.001)。两组间的ALFF值未发现显著差异。CD患者左侧中央内侧核、右侧基底外侧核、右侧浅表核的Reho值相比健康对照组降低(P<0.05)。双侧基底外侧核间、右侧浅表核与右侧中央内侧核、右侧浅表核与右侧基底外侧核间的Reho值具有显著正相关性(P<0.05)。结论:CD病患者左侧中央内侧核、右侧基底外侧核、右侧浅表核的局部一致性降低,并且杏仁核部分亚区具有显著相关性,提示CD患者情绪障碍和杏仁核亚区的功能紊乱有关,补充了脑肠轴紊乱的神经病理学机制,为脑肠轴的研究提供新的思路。 Purpose:To investigate changes in the level of spontaneous activity and local consistency of the amygdala subregion in patients with Crohn's disease(CD).Methods:Thirty CD patients and 30 healthy,age-and education-matched controls underwent functional magnetic resonance imaging(fMRI)and completed the hospital anxiety and depression scale(HADS).The REST toolkit was used to calculate amplitude of low frequency fluctuations(ALFF)and regional homogeneity(Reho)values for each amygdala subregion.The two-sample t test was used in SPSS 20.0 software to compare ALFF and Reho values of the three amygdala subregions,including basolateral nucleus,medial central nucleus,and superficial nucleus.Results:The HADS-A and HADS-D scores of CD patients were significantly higher than those of healthy controls(P<0.001).There was no significant difference in ALFF values between the two groups.Reho values in the left medial central nucleus,right basolateral nucleus,and right superficial nucleus were lower in CD patients than those in healthy controls(P<0.05).The Reho values of bilateral basolateral nuclei,right superficial nuclei and right medial central nuclei,right superficial nuclei and right basolateral nuclei were positively correlated(P<0.05).Conclusions:CD patients show decreased Reho values in the left medial central nucleus,the right basolateral nucleus and the right superficial nucleus,with significant correlation in some subregions of the amygdala.It suggests a link between mood disorders and functional disruption of the amygdala subregions in CD patients and supplements the neuro-pathological mechanisms of the brain-gut axis disorder and provides new insights for research on the brain-gut axis.
