摘要
目的 探讨医联体下糖尿病视网膜病变患者规范化综合管理模式的实施效果.方法 将2021年5月-2022年5月杭州市拱墅区朝晖街道社区卫生服务中心登记的180例糖尿病视网膜病变患者以数字抽签法分为干预组(90例)与对照组(90例),对照组实施常规管理模式,干预组在对照组基础上实施医联体下规范化综合管理模式,对两组管理12个月后血糖水平、视功能及管理期间上级医院就诊情况予以比较.结果 干预组管理后空腹血糖(FPG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbA1c)水平明显较对照组低,干预组管理后最佳矫正视力、视野平均缺失、视野指数情况明显优于对照组,干预组管理期间上级医院就诊频率、就诊时间明显少于对照组,上述指标差异均有统计学意义(P<0.05).结论 医联体下规范化综合管理模式能提高糖尿病视网膜病变患者血糖控制效果,改善患者视功能障碍,并能减少患者上级医院就诊频次与时间.
Objective To investigate the implementation effect of the standardized and comprehensive management model for patients with diabetic retinopathy under the medical alliance.Methods A total of 180 patients with diabetic retinopathy registered in the Community Health Service Center of Zhaohui Street,Gongshu District,Hangzhou from May 2021 to May 2022 were divided into intervention group(90 cases)and control group(90 cases)by digital lottery.Results The levels of fasting blood glucose(FPG),2 hour postprandial blood glucose(2hPG)and glycosylated hemoglobin(HbAlc)in the intervention group were significantly lower than those in the control group,and the best-corrected visual acuity,average loss of visual field and visual field index in the intervention group were significantly better than those in the control group,and the frequency and duration of visits to higher-level hospitals in the intervention group were significantly less than those in the control group,and the differences in the above indexes were statistically significant(P<0.05).Conc lusion The standardized and comprehensive management model under the medical alliance can improve the effect of blood glucose control in patients with diabetic retinopathy,improve the visual dysfunction of patients,and reduce the frequency and time of patients in higher-level hospitals.
