摘要
目的探索在临床上采用个性化复合式小梁切除术治疗青光眼及术后强化管理的有效性和安全性。方法回顾性病例研究。收集2020年1月至2022年10月在铜陵市人民医院眼科住院的青光眼患者63例(66只眼),均实施个性化复合式小梁切除术,术后给予强化管理,观察及对比术前术后视力、眼压、前房、滤过泡情况及并发症情况。结果术后随访至少3个月,术后1周、1个月、3个月视力与术前比较均有提升,差异均具有统计学意义(均P<0.05),术后1 d、1周、1个月、3个月眼压均较术前降低,差异均有统计学意义(均P<0.05),66只眼有55只眼视力提高,占83.33%,59只眼保持功能性滤过泡,眼压控制良好,占89.39%,有10只眼浅前房,占15.15%,术后均无严重并发症。结论青光眼患者采用个性化复合式小梁切除术及术后强化管理,效果确切,并发症少,可操作性强,成本低,推荐地市级及基层医院临床广泛开展。
Objective To explore the effectiveness and safety of individualized combined trabeculectomy and postoperative intensive management in the clinical treatment of glaucoma.Methods A retrospective case series study.A to⁃tal of 63 patients(66 eyes)of glaucoma treated in the department of ophthalmology of Tongling People's Hospital from Janu⁃ary 2020 to October 2022 were collected,all of whom underwent individualized combined trabeculectomy and were given postoperative intensive management.The terms of visual acuity,intraocular pressure,anterior chamber,filtering bleb,and complications were observed and compared before and after the surgery.Results After a follow-up period of at least 3 months,the visual acuity at 1 week,1 month,and 3 months after the surgery was improved compared with that before the surgery(P<0.05).The intraocular pressure at 1 day,1 week,1 month,and 3 months after the surgery was decreased com⁃pared with that before the surgery(P<0.05).Improvement of visual acuity was observed in 55 of 66 eyes(83.33%),59 eyes(89.39%)maintained functional filtering bleb and intraocular pressure was well controlled,10 eyes(15.15%)had shallow anterior chamber,and none had severe complications after the surgery.Conclusions The use of individualized combined trabeculectomy and intensive postoperative management in patients with glaucoma results in definitive outcomes,with few complications,great operability,and low cost.It is recommended to widely carry out in municipal and grassroots hospitals.
