摘要
目的:探讨关节镜下自体腘绳肌腱及横穿钉固定重建术治疗膝关节前交叉韧带断裂的临床效果。方法:选取2017年1月—2022年12月北京丰台医院收治的93例膝关节前交叉韧带断裂患者作为研究对象,随机分为观察组(n=48)和对照组(n=45)。对照组采用关节镜下传统技术重建前交叉韧带术治疗,观察组实施关节镜下自体腘绳肌腱及横穿钉固定重建术治疗。比较两组膝关节功能、膝关节前后向稳定度。结果:术前,两组国际膝关节文献委员会膝关节主观功能评估表(IKDC)、膝关节评分表(Lysholm)、运动水平评分表(Tegner)评分比较,差异无统计学意义(P>0.05);术后6个月,两组IKDC、Lyshohm、Tegner评分高于术前,且观察组高于对照组,差异有统计学意义(P<0.05)。术前,两组屈曲30°、90°时胫骨股骨间移动距离比较,差异无统计学意义(P>0.05);术后6个月,两组屈曲30°、90°时胫骨股骨间移动距离短于术前,且观察组短于对照组,差异有统计学意义(P<0.05)。结论:关节镜下自体腘绳肌腱及横穿钉固定重建术治疗膝关节前交叉韧带断裂的临床效果显著,能够提高患者膝关节功能及稳定性。
Objective:To investigate the clinical effect of arthroscopic autologous hamstring tendon and transfixion reconstruction in the treatment of anterior cruciate ligament fracture of knee joint.Methods:Ninety-three patients with anterior cruciate ligament fracture of knee joint admitted to Beijing Fengtai Hospital from January 2017 to December 2022 were selected as the study objects,and were randomly divided into observation group(n=48)and control group(n=45).The control group was treated with arthroscopic reconstruction of anterior cruciate ligament,and the observation group was treated with arthroscopic reconstruction of autogenous hamstring tendon and transfixion fixation.The knee function and anteroposterior stability were compared between the two groups.Results:Before surgery,there was no significant difference between the two groups on international knee literature committee subjective knee function assessment table(IKDC),knee joint rating(Lysholm)and exercise level rating(Tegner)scores(P>0.05);Six months after surgery,IKDC,Lysholm and Tegner scores in 2 groups were higher than before surgery,and observation group was higher than control group,the difference was statistically significant(P<0.05);Before surgery,there was no significant difference in the movement distance between the two groups at 30°and 90°flexion(P>0.05);At 6 months after surgery,the movement distance between the two groups at 30°and 90°flexion was shorter than that before surgery,and the observation group was shorter than the control group,the difference was statistically significant(P<0.05).Conclusion:Arthroscopic autologous hamstring tendon and transfixion reconstruction in the treatment of anterior cruciate ligament fracture of the knee has significant clinical effect,which can improve the function and stability of the patient's knee joint.
