摘要
目的探析肾阴不足型非增殖性糖尿病性视网膜病变患者采用真武汤加减治疗的效果及对视网膜血流动力的影响。方法选取2021年5月至2022年5月九江市中医医院收治的66例肾阴不足型非增殖性糖尿病性视网膜病变患者作为研究对象,按照随机数字表法分为对照组(33例)与观察组(33例),对照组采用羟苯磺酸钙胶囊治疗,观察组采用真武汤加减治疗。比较两组患者的血糖水平、24 h尿蛋白、血浆内皮素(ET)、血管内皮依赖性舒张功能及视网膜血流动力学指标。结果治疗前,两组患者血糖水平、24 h的尿蛋白含量比较,差异无统计学意义(P>0.05);治疗后,两组患者各指标水平低于本组治疗前,且观察组患者的血糖和24 h的尿蛋白低于对照组,差异有统计学意义(P<0.05)。治疗前,两组患者ET、血管内皮依赖性舒张功能比较,差异无统计学意义(P>0.05);治疗后,两组患者ET低于本组治疗前,血管内皮依赖性舒张功能高于本组治疗前,且观察组ET低于对照组,血管内皮依赖性舒张功能高于对照组,差异有统计学意义(P<0.05)。观察组治疗后视力、视力指数(RI)高于对照组、视野灰度值、收缩期最大流速(PSV)、舒张末期流速(EDV)低于对照组,差异有统计学意义(P<0.05);两组患者的最佳矫正视力(BCVA)、视网膜厚度的时间、组间、交互作用比较,差异有统计学意义(P<0.05)。结论真武汤加减治疗后,肾阴不足型非增殖性糖尿病性视网膜病变患者的血糖水平改善明显,其视网膜血流动力学指标水平趋于正常,患者ET、血管内皮舒张功能得到改善,临床上可借鉴及推广。
Objective To explore the effect of Zhenwu Decoction on the retinal blood flow dynamics in patients with non-proliferative diabetic retinopathy with renal Yin deficiency.Methods A total of 66 patients with non-proliferative diabetic retinopathy with renal Yin deficiency treated in Jiujiang Hospital of Traditional Chinese Medicine from May 2021 to May 2022 were selected as the study objects,and they were divided into control group(33 cases)and observation group(33 cases)according to random number table method.The control group was treated with Calcium Hydroxybenzenesulfonic Acid Capsules,and the observation group was treated with Zhenwu Decoction.The blood glucose level,24 h urinary protein,plasma endothelin(ET),endothelium dependent relaxation function and retinal hemodynamics were compared between the two groups.Results Before treatment,there were no significant differences in blood glucose level and 24 h urinary protein content between two groups(P>0.05).After treatment,the levels of all indexes in the two groups were lower than those before treatment,and the blood glucose and 24 h urinary protein in the observation group were lower than those in the control group,with statistical significances(P<0.05).Before treatment,there were no significantly differences in plasma ET and vascular endothelium-dependent relaxation function between two groups(P>0.05).After treatment,ET in two groups were lower than those before treatment,and vascular endothelium-dependent relaxation function were higher than those before treatment,and ET in observation group was lower than that of control group,and vascular endothelium-dependent relaxation function was higher than that of control group,with statistical significances(P<0.05).After treatment,visual acuity and resistance index(RI)in observation group were higher than those in control group,visual field gray value,peak systolic velocity(PSV)and end diastolic velocity(EDV)in observation group were lower than those in control group,the differences were statistically significant(P<0.0
