PNF绳带训练联合重复经颅磁刺激对脑卒中偏瘫患者上肢运动功能及神经功能的影响

Effects of PNF Rope Training Combined with Repetitive Transcranial Magnetic Stimulation on Upper Limb Motor Function and Neurological Function in Stroke Patients with Hemiplegia

摘要 目的:研究本体感觉神经肌肉促进(proprioceptive neuromuscular facilitation,PNF)绳带训练联合重复经颅磁刺激(repeat transcranial magnetic stimulation,rTMS)对脑卒中偏瘫患者上肢运动功能及神经功能的影响。方法:选取广州市中西医结合医院康复科2021年7月—2022年11月收治的脑卒中偏瘫患者150例,随机分为两组,对照组(75例)予以PNF绳带训练,研究组(75例)在此基础上予以r TMS。观察两组干预效果、上肢功能、神经功能。结果:研究组的临床总有效率高于对照组(P<0.05)。干预后,两组Fugl-Meyer评定量表上肢部分(Fugl-Meyer assessment-upper extremity,FMA-UE)评分、手臂动作调查测试评估量表(action research arm test,ARAT)评分均升高,且研究组均高于对照组,差异均有统计学意义(P<0.05)。干预后,两组美国国立卫生研究院卒中量表(national institute of health stroke scale,NIHSS)评分均降低,且研究组低于对照组,差异均有统计学意义(P<0.05);两组运动诱发电位波幅峰值均升高,且研究组高于对照组,差异均有统计学意义(P<0.05)。结论:PNF绳带训练联合rTMS应用于脑卒中偏瘫患者,干预效果较好,可改善患者上肢运动功能,促进患者神经功能恢复。 Objective:To study the effect of proprioceptive neuromuscular facilitation(PNF)rope training combined with repetitive transcranial magnetic stimulation(rTMS)on upper limb motor function and neurological function in stroke patients with hemiplegia.Method:A total of 150 stroke patients with hemiplegia admitted to the Department of Rehabilitation of Guangzhou Hospital of Integrated Traditional and Western Medicine from July 2021 to November 2022 were selected,randomly divided into two groups,the control group(75 cases)were trained with PNF rope straps,and the study group(75 cases)was received rTMS on this basis.The intervention effect,upper limb function,and neurological function of the two groups were observed.Result:The total clinical effective rate of the study group was higher than that of the control group(P<0.05).After the intervention,the Fugl-Meyer assessmentupper extremity(FMA-UE)scores and the action research arm test(ARAT)scores were increased in both groups,and those in the study group were higher than those in the control group,the differences were statistically significant(P<0.05).After the intervention,the scores of the national institute of health stroke scale(NIHSS)were significantly reduced in both groups,and that in the study group was lower than that in the control group,the differences were statistically significant(P<0.05);the peak amplitude of motion-evoked potential amplitude were increased in both groups,and that in the study group was higher than that in the control group,the differences were statistically significant(P<0.05).Conclusion:PNF rope training combined with rTMS in stroke patients with hemiplegia has a good intervention effect,which can improve the motor function of the upper limb and promote the recovery of the patient's neurological function.
