摘要
目的探讨并总结儿童膀胱尿路上皮肿瘤的诊断、治疗及病理特点。方法回顾性分析南京医科大学附属儿童医院自2015年1月至2021年9月收治的5例膀胱尿路上皮肿瘤患儿的临床资料,其中男3例,女2例;平均年龄为10.9岁,范围在6岁10个月至13岁10个月。5例患儿中4例因肉眼血尿就诊,1例体检时发现膀胱肿物。肿瘤位于膀胱三角区1例,膀胱前壁2例,膀胱左后壁1例,右侧输尿管开口上方1例。2例行经尿道膀胱肿瘤电切术,2例行开放式膀胱肿瘤剜除术,1例行气膀胱腹腔镜下膀胱肿瘤切除术。结果所有患儿均顺利完成手术,无术后并发症。患儿术后病理检查结果为:低度恶性潜能的尿路上皮乳头状瘤2例,内翻性尿路上皮乳头状瘤2例,尿路上皮乳头状瘤1例。5例患儿术后均获随访,平均随访31个月,范围在6~84个月。随访过程中未见肿瘤复发及转移。结论儿童膀胱尿路上皮肿瘤以非浸润型为主,大多为良性或低级别恶性肿瘤。治疗多采用经尿道膀胱肿瘤电切术,部分肿瘤需行膀胱肿瘤剜除术或膀胱部分切除术,大多预后良好。
Objective To explore the diagnosis,treatment and pathologic features of bladder urothelial tumor in children.Methods From January 2015 to September 2021,the relevant clinical data were retrospectively reviewed for 5 children with bladder urothelial tumor.There were 3 boys and 2 girls with an age range of 82 to 166 months.Four children were hospitalized for gross hematuria while another child revealed bladder mass during physical examination.Tumors were located in trigone of bladder(n=1),anterior wall of bladder(n=2),left posterior wall of bladder(n=1)and above right ureteral orifice(n=1).Transurethral resection(n=2),open resection(n=2)and pneumovesical resection(n=1)were performed.Results All procedures were successfully completed without any postoperative complication.The results of pathological examination were low malignant potential papillary urothelial neoplasm(n=2),inverted urothelial papilloma(n=2)and urothelial papilloma(n=1).During an average follow-up period of 31(6-84)months,there was no tumor recurrence or metastasis.Conclusions Bladder urothelial tumor in children are the mostly non-infiltrative benign or low-grade neoplasms.Transurethral resection is a first choice.And some times tumor enucleation or partial cystectomy is required.
