摘要
目的 了解北京市通州区0~3岁儿童发育筛查状况及影响因素,为早期制定干预措施提供参考。方法 通过北京市妇幼保健网络信息系统收集出生日期为2020年9月1日—2022年8月31日,且2023年9月1日前在通州区社区卫生服务中心进行丹佛发育筛查测验(DDST)的儿童资料,包括基本出生情况、母亲情况和DDST结果,分析DDST阳性率;采用多因素logistic回归模型分析DDST阳性的影响因素。结果 纳入0~3岁儿童27 783人,其中男童14 320人,占51.54%;女童13 463人,占48.46%。年龄M (QR)为0.91 (0.71)岁。早产2 471人,占8.89%。母亲妊娠期合并健康问题2 248人,占8.09%。DDST阳性儿童625例,阳性率为2.25%。多因素logistic回归分析结果显示,性别(男,OR=1.228,95%CI:1.045~1.444)、早产(OR=2.417,95%CI:1.890~3.092)、低出生体重(OR=2.583,95%CI:1.980~3.371)和母亲妊娠期合并健康问题(OR=2.012,95%CI:1.591~2.545)是0~3岁儿童DDST阳性的影响因素。结论 通州区0~3岁儿童DDST阳性率为2.25%,与性别、早产、低出生体重和母亲妊娠期合并健康问题有关。
Objective To learn the status and influencing factors of 0-3-year-old children's development screening in Tongzhou District,Beijing Municipality,so as to provide the reference for early intervention measures.Methods Data of children who were born from September 1,2020 to August 31,2022 and had received Denver Developmental Screen⁃ing Test(DDST)in Tongzhou District Community Health Service Center before September 1,2023 were collected through the Beijing Maternal and Child Healthcare Information System,including basic birth information,mother's situa⁃tion and DDST results.The screening positive rates of DDST among children were analyzed,and factors affecting posi⁃tive DDST were identified using a multivariable logistic regression model.Results A total of 27783 children aged 0-3 years were included,with a median age of 0.91(interquartile range,0.71)years.There were 14320 boys(51.54%)and 13463 girls(48.46%).There were 2471 preterm births(8.89%)and 2248 mothers with health problems during pregnancy(8.09%).The positive rate of DDST was 2.25%.Multivariable logistic regression analysis showed that gender(boy,OR=1.228,95%CI:1.045-1.444),preterm birth(OR=2.417,95%CI:1.890-3.092),low birth weight(OR=2.583,95%CI:1.980-3.371)and maternal health problems during pregnancy(OR=2.012,95%CI:1.591-2.545)were factors af⁃fecting positive DDST among children at ages of 0 to 3 years.Conclusions The positive rate of DDST among children at ages of 0 to 3 years was 2.25%in Tongzhou District.Gender,preterm birth,low birth weight and maternal health problems during pregnancy may affect positive screening for developmental delay.
