潮州地区自发性脑室出血患者脑脊液中蛋白含量与交通性脑积水的相关性分析

摘要 目的探讨潮州地区自发性脑室出血患者脑脊液中蛋白含量与交通性脑积水的相关性。方法选择2020年6月—2023年3月本院神经外科接受治疗的潮州地区自发性脑室出血患者50例为研究对象,通过腰椎穿刺术留取脑脊液,已行腰大池引流术或脑室外引流术的从中留取脑脊液送检查,记录患者发病后第7d、第14d脑脊液蛋白含量。根据测定结果将第7d脑脊液蛋白<800mg/L为A组、≥800mg/L为B组;将B组患者第14d脑脊液蛋白<800mg/L为B1组,≥800mg/L为B2组;发病后至14d,通过CT或MRI检查评估是否发生脑积水,比较各组交通性脑积水发生率,并完成相关性分析。结果50例自发性脑室出血患者,第7d脑脊液蛋白含量<800mg/L共17例,发病后至14d发生交通性脑积水0例;≥800mg/L患者共33例;其中第14d脑脊液蛋白<800mg/L患者共11例,发生交通性脑积水1例;第14d脑脊液蛋白≥800mg/L患者共22例;发生交通性脑积水共8例。B2组潮州地区自发性脑室出血患者交通性脑积水发生率为36.4%高于A组、B组和B1组(P<0.05);B组交通性脑积水发生率高于A组(P<0.05);A组和B1组交通性脑积水发生率无统计差异(P>0.05);Pearson相关性结果表明,潮州地区自发性脑室出血患者脑脊液中蛋白含量与交通性脑积水发生率呈正相关性(P<0.05)。结论潮州地区自发性脑室出血患者脑脊液中蛋白含量与交通性脑积水发生率呈正相关性,加强其表达水平测定能指导临床诊疗,预防交通性脑积水的发生。 Objective To explore correlation between protein content in cerebrospinal fluid and occurrence of communicating hydrocephalus of patients with spontaneous ventricular hemorrhage in Chaozhou area.Methods The paper chose 50 patients with spontaneous ventricular hemorrhage in Chaozhou area in neurosurgery department of our hospital from June 2020 to March 2023 as study subjects,collected cerebrospinal fluid through lumbar puncture,and cerebrospinal fluid from those with lumbar cistern drainage or external ventricular drainage for examination,recorded protein content of cerebrospinal fluid on the 7th and 14th days after onset of the disease.According to measurement results,cerebrospinal fluid protein<800mg/L on the 7th day was classified as group A,and≥800mg/L as group B.Group B with cerebrospinal fluid protein<800mg/L on the 14th day were classified as group B1,and patients with cerebrospinal fluid protein≥800mg/L as group B2.After 14 days of onset,incidence of hydrocephalus through CT or MRI examination was evaluated,incidence of communicating hydrocephalus in each group was compared,and correlation analysis was completed.Results Among 50 patients with spontaneous ventricular hemorrhage,17 had cerebrospinal fluid protein levels<800mg/L on the 7th day,and 0 had communicating hydrocephalus 14th day after onset,33 patients with 800mg/L;among them,there were 11 patients with cerebrospinal fluid protein<800mg/L on the 14th day,and 1 patient with communicating hydrocephalus;22 patients with cerebrospinal fluid protein≥800mg/L on the 14th day;8 cases of traffic induced hydrocephalus.Incidence of communicating hydrocephalus of patients with spontaneous ventricular hemorrhage in Chaozhou area of group B2 was 36.4%,higher than group A,group B,and group B1(P<0.05).Incidence of traffic induced hydrocephalus in group B was higher than group A(P<0.05).There was no statistical difference in incidence of traffic induced hydrocephalus between group A and B1(P>0.05).Pearson correlation results indicated there was positive corr
