摘要
目的探讨脑膜瘤患者术后静脉血栓栓塞症(Venous thrombus embolism,VTE)形成的风险因素,构建VTE形成列线图模型并进行验证。方法选取2020年1月-2022年12月新疆医科大学第一附属医院神经外科接受手术治疗的417例脑膜瘤患者为研究对象,采用单因素分析和多因素Logistic回归分析筛选417例脑膜瘤患者术后发生VTE的影响因素,构建列线图模型,并对模型进行内部验证。结果417例患者中150例(36%)发生了VTE,Logistic回归分析显示,年龄大于60岁、住院天数、D-二聚体、术后血钠、低密度脂蛋白为独立风险因素(P<0.05)。基于上述独立风险因素所构建的列线图模型,校准度(χ^(2)=6.424,P=0.781)和区分度[曲线下面积(Area under curve,AUC)=0.841,95%CI:0.800~0.882]良好。校准曲线与决策曲线显示,模型一致性及获益良好。结论以年龄大于60岁、住院天数、D-二聚体、术后血钠、低密度脂蛋白构建的脑膜瘤患者术后VTE发生的列线图模型具有较高的预测价值,可为脑膜瘤患者术后VTE的护理措施提供依据。
Objective To explore the risk factors for venous thromboembolism(VTE)in patients with meningioma surgery,construct a venous thrombosis risknomograph model and verify it.Methods 417 patients with postoperative meningiomas in the hospital from January 2020 to December 2022 were selected as research subjects,and single factor analysis and multivariate logistic regression analysis were used to screen the occurrence of postoperative meningiomas in patients with neurosurgery.Factors affecting VTE,build a risknomograph model,and conduct internal verification of the model.Results 150 of 417 patients(36%)developed venous thromboembolism.Logistic regression analysis showed that age older than 60 years,length of hospital stay,D-dimer,postoperative blood sodiumand low-density lipoprotein were independent risk factors(P<0.05).The column chart model constructed based on the independent risk factors mentioned above,calibration degree(χ^(2)=6.424,P=0.781)and discrimination[Area under curve(AUC)=0841,95%CI:0.800-0.882]was Good.The calibration curve and decision curve showed good model consistency and benefits.Conclusion A column chart model constructed with age greater than 60 years,length of hospital stay,D-dimer,postoperative blood sodium,and low-density lipoprotein has high predictive value for postoperative VTE in meningioma patientsthat can be constructed and has high predictive value and provides a theoretical basis for nursing measures for venous thromboembolism in patients with meningioma after surgery.
