摘要
目的 探讨个性化术中配合对骨肉瘤假体置换术患者术后应激反应与并发症的影响。方法 将120例接受假体置换术治疗的骨肉瘤患者按术中配合措施的不同分为A组和B组,每组60例。A组患者接受规范术中配合,B组患者接受个性化术中配合。对比两组患者血流动力学指标、心理状态、应激反应指标、手术相关指标及术后并发症发生情况。结果 术中,B组患者平均动脉压、心率均明显低于A组患者,差异均有统计学意义(P﹤0.01)。术后1天,B组患者焦虑自评量表(SAS)、抑郁自评量表(SDS)评分均明显低于A组患者,差异均有统计学意义(P﹤0.01)。术后1天,B组患者去甲肾上腺素、多巴胺、肾上腺素水平均低于A组患者,差异均有统计学意义(P﹤0.05)。B组患者手术时间、术后首次排气时间、住院治疗时间均明显短于A组患者,差异均有统计学意义(P﹤0.01)。B组患者并发症总发生率低于A组患者,差异有统计学意义(P﹤0.05)。结论 骨肉瘤患者接受假体置换术治疗时辅以个性化术中配合措施,能够确保血流动力学稳定,进而缩短手术时间,且明显降低患者心理及生理应激反应,有助于强化术后并发症控制效果,并加速术后康复,应用价值显著,值得重视及推广。
Objective To investigate the effect of personalized intraoperative cooperation on postoperative stress response and complications in patients undergoing osteosarcoma prosthesis replacement.Method A total of 120 patients with osteosarcoma who underwent osteosarcoma prosthesis replacement were divided into group A and group B according to different intraoperative cooperation measures,with 60 cases in each group.Patients in group A received standardized intraoperative cooperation,and patients in group B received personalized intraoperative cooperation.Hemodynamic indicators,psychological status,stress response indicators,surgery-related indicators,and postoperative complications were compared between the two groups.Result During the operation,the mean arterial pressure and heart rate of patients in group B were significantly lower than those in group A,and the differences were statistically significant(P<0.01).One day after the surgery,the self-rating anxiety scale(SAS) and self-rating depression scale(SDS) scores in group B were significantly lower than those in group A,and the differences were statistically significant(P<0.01).One day after the surgery,the levels of norepinephrine,dopamine,and epinephrine in group B were lower than those in group A,and the differences were statistically significant(P<0.05).The operation time,postoperative first exhaust time,and hospitalization time in group B were significantly shorter than those in group A,and the differences were statistically significant(P<0.01).The total incidence of complications in group B was lower than that in group A,and the difference was statistically significant(P<0.05).Conclusion When patients with osteosarcoma undergoing osteosarcoma prosthesis replacement,personalized intraoperative coordination measures could ensure hemodynamic stability and shorten the operation time.It could also significantly reduce both patients' psychological and physiological stress responses,and help to strengthen the control effect of postoperative complications and acceler
