摘要
目的比较经肘前横纹入路与肘部外侧入路在治疗小儿GartlandⅢ型肱骨髁上骨折疗效。方法选取2021年5月—2022年8月自贡市第四人民医院于收治的GartlandⅢ型肱骨髁上骨折患儿84例为研究对象,采用抽签法将其分为对照组和观察组,各42例。对照组采用肘部外侧入路,观察组采用经肘前横纹入路,对比两组患儿的手术相关指标、肘关节功能情况及并发症发生率。结果观察组切口显露时间、手术时间及肘关节功能恢复时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);两组患儿切口感染、肘关节过伸、骨化性肌炎、神经损伤等并发症发生率比较,差异无统计学意义(P>0.05);观察组肘关节功能恢复优良率(95.24%)高于对照组,差异有统计学意义(χ^(2)=0.578,P<0.05)。结论经肘前横纹入路方式临床效果良好,相较于肘部外侧入路,患儿肘关节功能恢复时间更快。
Objective To compare the efficacy of transcubital transverse approach and lateral elbow approach in the treatment of supracondylar fracture of humerus Gartland type III in children.Methods A total of 84 children with Gartland type III supracondylar fracture of humerus admitted to the of Zigong Fourth People's Hospital from May 2021 to August 2022 were selected as the study objects.They were divided into control group and observation group by drawing lots,with 42 cases in each group.The control group was treated with the lateral elbow approach,and the observation group was treated with the transcubital transverse approach.The operative indexes,elbow joint function and complication rate of the two groups were compared.Results The incision exposure time,operation time and elbow joint function recovery time in the observation group were shorter than those in the control group,and the intraoperative blood loss was less than that in the control group,and the differences were statistically significant(P<0.05).There was no statistically significant difference in incision infection,elbow hyperextension,myositis ossificans and nerve injury between the two groups(P>0.05).The excellent and good rate of elbow joint function recovery in the observation group(95.24%)was higher than that in the control group,and the difference was statistically significant(χ^(2)=0.578,P<0.05).Conclusion Compared with the lateral elbow approach,the clinical effect of transcubital transverse approach is good,the elbow joint function recovery time is faster.
