摘要
目的探讨血清碱性成纤维细胞生长因子(bFGF)联合基质细胞衍生因子-1α(SDF-1α)对断指再植术后血运情况的评估价值。方法回顾性分析2021年3月—2022年3月医院收治的72例断指再植术后患者的临床资料,术后随访1年,根据患者早期预后分为血运异常组(n=25)与非血运异常组(n=47)。比较血运异常组与非血运异常组血清bFGF、SDF-1α水平,比较血运异常组与非血运异常组临床资料,Logistic多因素回归分析断指再植术后血运异常的影响因素,绘制受试者工作特征曲线(ROC),以曲线下面积(AUC)评估血清bFGF、SDF-1α水平及二者对断指再植术后血运异常的评估价值。结果断指再植术后出现异常25例,记为血运异常组,非血运异常47例,记为非血运异常组。血运异常组血清bFGF水平高于非血运异常组(P<0.05),血运异常组术前SDF-1α水平低于非血运异常组(P<0.05)。血运异常组与非血运异常组,年龄、性别、吸烟、合并高血压、合并糖尿病、合并高血脂、附睾蛋白4(HE4)比较,差异均无统计学意义(P>0.05)。以断指再植术后1年是否血运异常为因变量(非血运异常=0,血运异常=1),以血清bFGF、SDF-1α水平为自变量进行Logistic多因素回归分析,结果显示血清bFGF、SDF-1α均为断指再植术后血运异常的影响因素(P<0.05),ROC曲线结果显示血清bFGF、SDF-1α水平及二者联合评估断指再植术后血运异常的AUC值(95%CI)分别为0.713、0.719、0.811(P<0.05)。结论血清bFGF、SDF-1α水平可用于评估断指再植术后血运异常,且二者联合的评估价值更高。
Objective To investigate the value of serum basic fibroblast growth factor(bFGF)combined with stromal cell derived factor-1α(SDF-1α)in the evaluation of blood circulation after replantation of severed finger.Methods The clinical data of seventy two patients admitted to the hospital from March 2021 to March 2022 after replantation of severed finger were retrospectively analyzed.The patients were followed up for 1 year and divided into abnormal blood circulation group(n=25)and non-abnormal blood circulation group(n=47)according to the early prognosis.The serum bFGF and SDF-1α levels of the abnormal blood circulation group and the non-abnormal blood circulation group were compared,the clinical data of the abnormal blood circulation group and the non-abnormal blood circulation group were compared,the influencing factors of abnormal blood circulation after finger replantation were analyzed by Logistic multivariate regression,and the receiver operating characteristic curve(ROC)was drawn.Area under the curve(AUC)was used to evaluate the value of serum bFGF and SDF-1α levels and their combination in the assessment of abnormal blood circulation after replantation of severed finger.Results 25 cases of abnormal finger replantation were recorded as abnormal blood circulation group and 47 cases were recorded as non-abnormal blood circulation group.The serum bFGF level in the abnormal blood circulation group was higher than that in the non-abnormal blood circulation group(P<0.05),and the preoperative SDF-1α level in the abnormal blood circulation group was lower than that in the non-abnormal blood circulation group(P<0.05).There were no significant differences in age,sex,smoking,hypertension,diabetes,hyperlipidemia and epididymis protein 4(HE4)between the abnormal blood circulation group and the non-abnormal blood circulation group(P<0.05).One year after replantation of severed finger,abnormal blood circulation was used as dependent variable(non-abnormal blood circulation=0,abnormal blood circulation=1),and serum bFGF and
