摘要
目的 分析前交叉韧带重建(ACLR)术后患者重返运动的促进与阻碍因素。方法 选取2022年4月至2023年1月在东部战区总医院骨科初次行ACLR手术治疗的患者。采用质性研究对患者进行半结构式访谈,分析促进及阻碍因素。结果 提炼出促进因素包括内控型人格(28.6%)、医护定期随访(21.4%)、外部环境助推(14.3%);阻碍因素包括康复认知低效、生理层面障碍、系统决策支持缺乏、再次损伤的恐惧、负性心理情绪突出。结论 前交叉韧带重建术后患者重返运动的影响因素是多维度的,医护人员应实施精准康复评估,及时解决躯体障碍,提供心理康复保障,制订基于患者运动康复偏好的护理方案,改善术后重返运动率。
Objective To analyze enablers and barriers of patients returning to exercise after anterior cruciate ligament reconstruction(ACLR).Methods From April 2022 to January 2023,it has been selected the patients who underwent ACLR surgery for the first time in the Department of Orthopedics of the Eastern Theater General Hospital.Semi-structured interviews were conducted with patients using qualitative research to analyze the enablersand barriers.Results Theenablers were internal control personality(28.6%),regular follow-up(21.4%)and external environment(14.3%).The barriers include cognitive inefficiency of rehabilitation,physiological barriers,lack of systematic decision support,fear of re-injury,and prominent negative psychological emotions.Conclusion The factors influencing the return to exercise of patients after anterior cruciate ligament reconstruction are multi-dimensional.Medical staff should implement accurate rehabilitation assessment,timely solve physical disorders,provide psychological rehabilitation support,formulate nursing programs based on patients'preference for exercise rehabilitation,and improve the rate of return to exercise after surgery.
