摘要
目的观察大柴胡汤合防己黄芪汤联合二甲双胍对肥胖型多囊卵巢综合征(PCOS)患者糖脂代谢以及性激素水平的影响。方法选取2021年5月至2022年2月在本院中医妇科门诊就诊的肥胖型PCOS患者60例。依据随机数字表法分成治疗组与对照组,每组各30例。对照组口服二甲双胍治疗,治疗组在对照组的基础上口服大柴胡汤合防己黄芪汤。连续治疗3个月后,比较两组患者治疗前后性激素、抗苗勒氏管激素(AMH)及糖脂代谢指标水平。结果治疗3个月后两组血清睾酮(T)、促黄体生成素(LH)、AMH、体质量、体质量指数(BMI)、体脂率、甘油三酯(TG)、总胆固醇(TC)水平均比治疗前下降(P<0.05),且治疗组下降更明显(P<0.05);两组治疗前与治疗后卵泡刺激素(FSH)水平均无明显差异(P>0.05)。治疗后两组空腹血糖(FPG)、空腹胰岛素(FINS)、1h血糖(1hPG)、1h胰岛素(1hINS)、2h血糖(2hPG)、2h胰岛素(2hINS)、3h血糖(3hPG)及3h胰岛素(3hINS)水平均比治疗前下降(P<0.05),且治疗组下降更明显(P<0.05)。结论大柴胡汤合防己黄芪汤联合二甲双胍治疗肥胖型PCOS效果显著,可有效改善糖脂代谢紊乱及性激素水平,提高排卵率,且不良反应小,值得临床推广。
Objective To observe the effects of dachaihu Decoction,Fangji Huangqi Decoction combined with metformin on glucose and lipid metabolism and sex hormone levels in obese polycystic ovary syndrome(PCOS).Methods From May 2021 to February 2022,60 obese PCOS patients were selected from the outpatient department of traditional Chinese medicine and Gynecology of the second people's Hospital of Haining City,Zhejiang Province.According to the random number table,the patients were divided into the treatment group and the control group,with 30 patients in each group.The control group was orally treated with metformin,and the treatment group was orally treated with dachaihu Decoction and Fangji Huangqi Decoction on the basis of the control group.After 3 months of continuous treatment,the levels of sex hormones,AMH and glycolipid metabolism were compared between the two groups before and after the treatment.Results After 3 months of the treatment,T,LH,AMH,body mass,BMI,body fat ratio,TG and TC of the two groups decreased compared with those before the treatment(P<0.05),and the decrease was more significant in the treatment group(P<0.05).There was no difference between the two groups before and after the FSH treatment(P>0.05).After the treatment,FPG,FINS,1 hPG,1 hINS,2 hPG,2 hINS,3 hPG and 3 hINS in both groups decreased compared with those before the treatment(P<0.05),and the decrease was more significant in the treatment group(P<0.05).Conclusion Dachaihu Decoction and Fangji Huangqi decoction combined with metformin have a significant effect on obese PCOS,which can effectively improve the disorder of glucose and lipid metabolism and the level of sex hormones,and increase the ovulation rate.The adverse reaction is small,which is worthy of clinical promotion.
