摘要
目的分析25-羟维生素D联合白细胞计数(WBC)、中性粒细胞/淋巴细胞比值(NLR)和血小板/淋巴细胞比值(PLR)对孕早期妊娠期糖尿病(GDM)的预测价值。方法选取2022年7月至2023年9月云浮市妇幼保健院妇产科门诊产检的孕早期GDM确诊孕妇147例为GDM组,同期非GDM孕妇101例为对照组。于孕早期12~13周进行25-羟维生素D和血常规测定,同时计算NLR和PLR,并对结果统计分析。结果GDM组25-羟维生素D水平为(14.40±4.51)ng/ml,低于非GDM组的(20.51±4.12)ng/ml(P=0.029);GDM组WBC、NLR及PLR水平分别为(10.78±2.55)×10^(9)/L、3.15±0.97及110.54±39.89,高于非GDM组的(8.64±2.47)×10^(9)/L、2.50±0.74及89.86±34.68(P=0.035、0.040、0.043)。Pearson相关分析显示,25-羟维生素D与WBC呈负相关(r=-0.865,P=0.027),而与NLR和PLR水平无相关性。ROC曲线分析显示,25-羟维生素D联合WBC、NLR及PLR检测对孕早期GDM诊断的AUC(95%CI)、特异度及灵敏度分别为0.901(95%CI 0.854~0.947)、0.755及0.902,优于上述指标单独检测。结论孕早期GDM患者25-羟维生素D水平降低,而WBC、NLR及PLR水平升高,且两者存在一定相关性。同时,联合检测对孕早期GDM诊断比上述指标单独检测具有更好的预测价值。
Objective To analyze the predictive value of 25-hydroxyvitamin D[25-(OH)VitD]combined with white blood cell count(WBC),neutrophil/lymphocyte ratio(NLR),and platelet/lymphocyte ratio(PLR)for gestational diabetes mellitus(GDM)in first trimester.Methods One hundred and forty-seven pregnant women with confirmed GDM in first trimester who underwent obstetrics and gynecology outpatient labor and delivery examination in Yunfu Maternal and Child Health Hospital from July 2022 to September 2023 were selected as the GDM group,and 101 non-GDM pregnant women during the same period were selected as the control group.The 25-(OH)VitD and blood routine measurements were carried out at 12-13 weeks of first trimester,and NLR and PLR were calculated at the same time,and the results were statistically analyzed.Results The 25-(OH)VitD level in the GDM group was(14.40±4.51)ng/ml,which was lower than that of(20.51±4.12)ng/ml in the non-GDM group(P=0.029);the levels of WBC,NLR and PLR in the GDM group were(10.78±2.55)×10^(9)/L,3.15±0.97,and 110.54±110.54 respectively.39.89,which were higher than(8.64±2.47)×10^(9)/L,2.50±0.74 and 89.86±34.68 in the non-GDM group(P=0.035,0.040,0.043).Pearson's correlation analysis showed a negative correlation between 25-(OH)VitD and WBC(r=-0.865,P=0.027),while there was no correlation with NLR and PLR levels.ROC curve analysis showed that the AUC(95%CI),specificity and sensitivity of 25-(OH)VitD combined with WBC,NLR and PLR tests for the diagnosis of GDM in first trimester were 0.901(95%CI 0.854-0.947),0.755 and 0.902,respectively,which were superior to the The above indicators were detected individually.Conclusions 25-(OH)VitD levels were decreased and WBC,NLR and PLR levels were increased in patients with GDM in first trimester,and there was a certain correlation between the two.Meanwhile,the combined test has a better predictive value for the diagnosis of GDM in first trimester than the above indicators alone.
