摘要
目的 分析全螺纹无头加压空心螺钉修复胫骨远端三平面骨折的临床疗效。方法 160例胫骨远端三平面骨折患者,随机分为观察组和对照组,每组80例。对照组患者采用解剖钢板螺钉固定治疗,观察组患者采取全螺纹无头加压空心螺钉修复治疗。比较两组患者手术指标、并发症发生情况以及关节功能恢复效果。结果 观察组患者切口长度(6.62±1.02)cm、手术时间(87.83±14.28)min、术后下地时间(11.63±2.29)周、骨折愈合时间(12.55±2.31)周均短于对照组的(9.87±1.84)cm、(115.24±18.95)min、(13.43±1.28)周、(15.48±2.52)周,术中出血量(57.00±21.17)ml、术后出血量(45.74±14.03)ml均少于对照组的(80.33±21.07)、(75.33±14.44)ml,差异均具有统计学意义(P<0.05)。观察组患者并发症发生率3.75%低于对照组的18.75%,差异具有统计学意义(P<0.05)。观察组患者关节功能恢复优良率98.75%高于对照组的66.25%,差异具有统计学意义(P<0.05)。结论 全螺纹无头加压空心螺钉修复治疗胫骨远端三平面骨折,可显著提升治疗效果,优化手术治疗指标,缩短患者的康复时间,降低并发症发生率,显著提升患者的关节功能恢复效果。
Objective To analyze the clinical efficacy of fully thread headless cannulated compression screws in the repair of tri plane fracture of distal tibia.Methods A total of 160 patients with tri plane fracture of distal tibia were randomly divided into an observation group and a control group,with 80 cases in each group.The control group received anatomical plate screw internal fixation,and the observation group received fully thread headless cannulated compression screws for repair. The surgical indicators, the occurrence of complications and the recovery effect of joint function were compared between the two groups. Results The incision length in the observation group was (6.62±1.02) cm, the operative time was (87.83±14.28) min, the postoperative ambulation time was (11.63±2.29) weeks and the fracture healing time was (12.55±2.31) weeks, which were all shorter than those of (9.87±1.84) cm, (115.24±18.95) m, (13.43±1.28) weeks and (15.48±2.52) weeks in the control group;the intraoperative blood loss of (57.00±21.17) ml and postoperative blood loss of (45.74±14.03) ml in the observation group were lower than those of (80.33±21.07) and (75.33±14.44) ml in the control group;the differences were statistically significant (P<0.05). The incidence of complications of 3.75% in the observation group was lower than that of 18.75% in the control group, and the difference was statistically significant (P<0.05). The excellent and good rate of joint function recovery in the observation group was 98.75%, which was higher than that of 66.25% in the control group, and the difference was statistically significant (P<0.05). Conclusion Fully thread headless cannulated compression screws for the treatment of tri plane fracture of distal tibia can significantly improve the therapeutic effect of patients, optimize the surgical treatment indicators, shorten the rehabilitation time of patients, reduce the incidence of complications, and significantly improve the recovery effect of joint function of patients.
