摘要
目的研究低剂量颅脑CT脑灌注成像(CT perfusion,CTP)参数与缺血性脑血管病患者颅内动脉、颈动脉狭窄程度和预后的关系。方法方便选取2019年1月—2021年1月盐城市第三人民医院收治的106例缺血性脑血管疾病患者为研究对象。分别对健侧以及患侧的低剂量颅脑CTP参数进行比较,对比不同颅内动脉狭窄、颈动脉狭窄、预后患者的局部脑血流量(cerebral blood flow,CBF)、局部脑血容积(cerebral blood volume,CBV)以及血液通过脑组织的平均通过时间(mean transit time,MTT)、达峰时间(time to peak,TTP)之间的差异,研究颅内动脉狭窄、颈动脉狭窄中低剂量颅脑CTP参数。结果患侧的CBF[(18.43±3.77)mL/(100 g·min)]、CBV[(2.03±0.89)mL/100 g]显著低于健侧,MTT[(8.82±2.89)s]、TTP[(16.43±2.65)s]显著高于健侧,差异有统计学意义(t=66.995、7.026、15.237、12.783,P<0.05);不同颅内动脉狭窄患者MTT、CBV、CBF及TTP水平比较,差异有统计学意义(P<0.05)。结论缺血性脑血管病患者颅内动脉、颈动脉狭窄程度的低剂量颅脑CTP参数可作为疾病诊断以及治疗效果评价的重要依据。
Objective To explore the relationship between low-dose cranial CT perfusion(CTP)parameters and the degree of stenosis and prognosis of intracranial artery and carotid artery in patients with ischemic cerebrovascular disease.Methods 106 patients with ischemic cerebrovascular disease admitted to Yancheng Third People's Hospital from January 2019 to January 2021 were conveniently selected as the study objects.The low-dose cranial CTP parameters of the healthy side as well as the affected side were compared,respectively,to compared the differences between the cerebral blood volume(CBV),cerebral blood flow(CBF),mean transit time(MTT)and time to peak(TTP)of patients with different intracranial stenosis,carotid stenosis,and prognosis,and to study the low-dose cranial CTP parameters in intracranial stenosis and carotid stenosis.Results The CBF[(18.43±3.77)mL/(100 g·min)]and CBV[(2.03±0.89)mL/100 g]on the affected side were lower than on the healthy side,and MTT[(8.82±2.89)s]and TTP[(16.43±2.65)s]were significantly higher than those on the healthy side,and the differences were statistically significant(t=66.995,7.026,15.237,12.783,P<0.05).The comparison between MTT,CBV,CBF and TTP levels in patients with different intracranial artery stenosis,and the differences were statistically significant(P<0.05).Conclusion Low-dose cranial CTP parameters of the degree of stenosis of intracranial arteries and carotid arteries in patients with ischemic cerebrovascular disease can be used as important reliance for the diagnosis of the disease as well as the evaluation of the therapeutic effect.
