摘要
目的探究Bobath康复疗法联合经颅直流电刺激治疗缺血性卒中后情感淡漠的疗效及对5-HT、BDNF的影响。方法选取2021年2月-2023年2月期间吉安市中心人民医院收治的缺血性脑卒中后情感淡漠患者94例,应用随机数字表法将其分为对照组(Bobath康复疗法)及观察组(Bobath康复疗法联合经颅直流电刺激),每组各47例,对比两组治疗有效率、HAMD、AES-C、5-HT、BDNF、NGF、IGF-1、MoCA、MMSE及不良反应情况。结果对照组治疗有效率低于观察组,差异有统计学意义(P<0.05)。治疗后,观察组HAMD、AES-C低于对照组,差异有统计学意义(P<0.05)。治疗后,对照组5-HT、BDNF、NGF水平低于观察组,IGF-1水平高于观察组,差异有统计学意义(P<0.05)。治疗后,观察组MoCA、MMSE评分高于对照组,差异有统计学意义(P<0.05)。对照组不良反应发生率与观察组比较,差异无统计学意义(P>0.05)。结论Bobath康复疗法联合经颅直流电刺激治疗缺血性卒中后情感淡漠效果显著,可改善患者淡漠情绪、认知水平、精神状态,提高神经营养因子水平,安全性较高。
Objective To explore the efficacy of Bobath rehabilitation therapy combined with transcranial direct current stimulation in the treatment of apathy after ischemic stroke and its influence on 5-HT and BDNF.Methods 94 patients with apathy after ischemic stroke admitted to our hospital from February 2021 to February 2023 were selected and divided into control group(Bobath rehabilitation therapy)and observation group(Bobath rehabilitation therapy combined with transcranial direct current stimulation)by random number table method,47 cases in each group.The effective rate,HAMD,AES-C,5-HT,BDNF,NGF,IGF-1,MoCA,MMSE and adverse reactions of the two groups were compared.Results The treatment effective rate of control group was lower than that of observation group,the difference was statistically significant(P<0.05).After treatment,HAMD and AES-C in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).After treatment,the level of 5-HT,BDNF and NGF in control group was lower than that in observation group,and the level of IGF-1 was higher than that in observation group,and the difference was statistically significant(P<0.05).After treatment,MoCA and MMSE scores in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in adverse reactions between control group and observation group(P>0.05).Conclusion Bobath rehabilitation therapy combined with transcranial direct current stimulation has a significant effect on the treatment of apathy after ischemic stroke,which can improve the apathy,cognitive level and mental state of patients and improve the level of neurotrophic factors,with high safety.
