摘要
目的探讨HP阳性慢性萎缩性胃炎(CAG)患者血清CD47、核转录因子-κB(NF-κB)及三叶因子2(TFF2)变化及意义。方法收集2021年2月至2023年2月间在上海交通大学附属仁济医院浦南分院收治且经胃镜、组织病理学活检确诊为慢性萎缩性胃炎患者228例作为研究对象,根据是否合并HP阳性分为HP阳性CAG组(n=93)、HP阴性CAG组(n=135)。另取同期在本院进行体检的健康志愿者100名作为正常对照组。对比三组研究对象的血清CD47、NF-κB及TFF2水平差异,分析HP阳性CAG患者血清中CD47水平与NF-κB、TFF2的相关性。根据Sydney系统将CAG患者的炎症反应程度分为1级(轻度)、2级(中度)、3级(重度),根据慢性胃炎OLGA评估系统将CAG患者的病变分期分为Ⅰ期、Ⅱ期、Ⅲ期、Ⅳ期,对比不同病情严重程度HP阳性CAG患者的血清CD47、NF-κB及TFF2水平差异。结果三组间血清CD47、NF-κB水平的比较为HP阳性CAG组>HP阴性CAG组>正常对照组,TFF2水平的比较为HP阳性CAG组2级>3级,差异均有统计学意义(F=27.135、26.833、21.618,P<0.05);不同分期的HP阳性CAG患者血清CD47、NF-κB水平的比较结果为Ⅰ期<Ⅱ期<Ⅲ期<Ⅳ期,TFF2水平的比较结果为Ⅰ期>Ⅱ期>Ⅲ期>Ⅳ期,差异有统计学意义(F=37.216、27.917、25.384,P<0.05)。结论HP阳性CAG患者的血清CD47、NF-κB水平异常增加,TFF2水平异常降低且与病变严重程度加重相关。
Objective To investigate the changes and significance of CD47,nuclear transcription factor-κB(NF-κB)and three-leaf factor 2(TFF2)in serum of HP-positive chronic atrophic gastritis(CAG).Methods A total of 228 patients admitted to Shanghai Jiaotong University Affiliated Renji Hospital Punan Branch from February 2021 to February 2023 who were diagnosed with chronic atrophic gastritis by gastroscopy and histopathological biopsy were collected as research objects and divided into the HP-positive CAG group(n=93)and the Hp-negative CAG group(n=135)according to whether they were combined with HP positive.Another 100 health volunteers who underwent physical examination in our hospital during the same period were selected as the normal control group.The serum levels of CD47,NF-κB and TFF2 in three groups were compared,correlation between serum levels of CD47 and NF-κB and TFF2 in patients with HP-positive CAG was analyzed.According to the Sydney system,degree of inflammatory response in patients with CAG was divided into grade 1(mild),grade 2(moderate)and grade 3(severe),according to OLGA evaluation system of chron-ic gastritis,pathological stages of patients with CAG were divided into stageⅠ,stageⅡ,stageⅢand stageⅣ.The serum levels of CD47,NF-κB and TFF2 in HP-positive CAG patients with different severity were compared.Results The comparison of serum CD47 and NF-κB levels among the three groups was HP-positive CAG group>HP-negative CAG group>normal control group,and the comparison of TFF2 levels was HP-positive CAG group<HP-negative CAG group<normal control group,the difference was statistically significant(F=165.938,189.372,131.713,P<0.05).Pearson test showed that the serum CD47 level in HP-positive CAG patients was positively correlated with NF-κB level(r=0.374)and negatively correlated with TFF2 level(r=0.391)(P<0.05).The comparison of serum CD47 and NF-κB levels in different grades of HP-positive CAG patients were grade 1<grade 2<grade 3,and the comparison results of TFF2 levels were grade 1>grade 2>gr
