钆塞酸二钠增强磁共振成像用于慢性乙型病毒性肝炎肝纤维化评估的临床价值

摘要 目的分析钆塞酸二钠(Gd-EOB-DTPA)增强磁共振成像(MRI)用于慢性乙型病毒性肝炎(CHB)肝纤维化评估的价值。方法收集2020年1月至2022年12月于江苏省句容市人民医院就诊、临床疑似肝脏病变接受Gd-EOB-DTPA增强MRI检查及肝穿刺并取得病理的CHB患者80例,依据病理分期:极少纤维化为B组(n=20)、轻度纤维化为C组(n=20)、中度纤维化为D组(n=20)、肝硬化为E组(n=20),另选同期无慢性肝病病史、肝功能正常、因其他疾病需行Gd-EOB-DTPA增强MRI检查患者20例作为A组,对比各组信号绝对增强强度(AEV)、对比增强率(CER),测量肝组织肝胆期增强扫描后的T_(1)值(T_(1)post),计算肝细胞分数(HeF)、摄取系数(KHeP),分析各指标对肝纤维化评估价值,Spearman相关分析AEV、CER、T_(1)post、HeF、KHeP与肝纤维化的关系。结果B、C、D、E组AEV、CER对比,B组数值最高,E组数值最低(P<0.05)。A、B、C、D、E组T_(1)post、HeF、KHeP对比,A组T_(1)post最低,E组T_(1)post最高,A组HeF、KHeP最高,E组HeF、KHeP最低(P<0.05)。AEV、CER、T_(1)post、HeF、KHeP各指标对肝纤维化评估敏感度、特异度、准确率均较高。AEV、CER、HeF、KHeP与肝纤维化程度均呈负相关(r=-0.864、-0.881、-0.825,P均<0.001),T_(1)post与肝纤维化程度呈正相关(r=0.895,P<0.001)。结论Gd-EOB-DTPA增强MRI对CHB肝纤维化评估价值较高。 Objective To analyze the value of disodium gadolinium selenate(Gd-EOB-DTPA)enhanced magnetic resonance imaging(MRI)for the evaluation of hepatic fibrosis(HF)in chronic hepatitis B(CHB).Methods A total of 80 hepatitis B patients with clinically suspected liver lesions who underwent Gd-EOB-DTPA enhanced MRI and liver puncture and obtained pathological results from January 2020 to December 2022 were collected.According to the pathological staging,the patients with minimal fibrosis for group B(n=20),mild fibrosis for group C(n=20),moderate fibrosis for group D(n=20),and cirrhosis for group E(n=20).Additionally,20patients with no history of chronic liver disease,normal liver function,and the need for Gd-EOB-DTPA-enhanced MRI due to other diseases in the same period were selected as group A.The absolute enhancement velocity(AEV)and contrast enhancement rate(CER)of signals in each groups were compared,and the T_(1) value(T_(1)post),calculated hepatocyte fraction(HeF),and uptake coefficient(KHeP)of liver tissue after hepatobiliary phase enhanced scanning were measured to analyze the value of each index for the evaluation of liver fibrosis,and Spearman correlation analysis was conducted to investigate the relationship between AEV,CER,T_(1)post,HeF,KHeP and liver fibrosis.Results Comparing the AEV and CER values of groups B,C,D,and E,group B had the highest value and group E had the lowest value(P<0.05).Comparing the T_(1)post,HeF,and KHeP values of groups A,B,C,D,and E,group A had the lowest T_(1)post,while group E had the highest T_(1)post.Group A had the highest HeF and KHeP values,and group E had the lowest HeF and KHeP values(P<0.05).The sensitivity,specificity,and accuracy of each index of AEV,CER,T_(1)post,HeF,and KHeP for the assessment of liver fibrosis were high.AEV,CER,HeF,KHeP were all negatively correlated with the degree of liver fibrosis(r=-0.864,-0.881,-0.825,P<0.001),while T_(1)post was positively correlated with the degree of liver fibrosis(r=0.895,P<0.001).Conclusion Gd-EOB-DTPA enhanced MRI has a high v
