定量磁共振成像检查诊断胰腺脂肪沉积及与2型糖尿病的相关性分析

摘要 目的:探讨定量磁共振成像(MRI)检查在胰腺脂肪沉积诊断中的应用价值,并分析胰腺脂肪沉积与2型糖尿病的相关性。方法:选取2020年6月—2023年6月兰州市第二人民医院收治的100例2型糖尿病患者作为观察组,选取同期体检健康者90名作为对照组。两组均进行定量MRI检查,并收集两组临床资料,比较两组胰腺脂肪含量、血糖、血脂、身体质量指数(BMI),分析胰腺脂肪含量和血糖、血脂、BMI的关系。结果:观察组各年龄胰腺平均脂肪分数,胰头、胰体与胰尾脂肪分数,空腹血糖(FPG)、低密度脂蛋白胆固醇(LDL-C)、甘油三酯(TG)、总胆固醇(TC)、身体质量指数(BMI)水平高于对照组,高密度脂蛋白胆固醇(HDL-C)水平低于对照组,差异有统计学意义(P<0.05)。2型糖尿病患者胰腺平均脂肪分数,胰头、胰体与胰尾脂肪分数与FPG、LDL-C、TG、TC、BMI呈正相关,与HDL-C呈负相关(P<0.05)。结论:2型糖尿病患者胰腺脂肪分数高于健康人群,其胰腺脂肪含量和血糖、血脂、BMI水平密切相关,说明胰腺脂肪沉积和2型糖尿病的疾病发展有重要联系,可能是2型糖尿病的危险因素。 Objective:To investigate the application value of quantitative MRI in the diagnosis of pancreatic fat deposition,and analyze the correlation between pancreatic fat deposition and type 2 diabetes mellitus.Methods:A total of one hundred patients with type 2 diabetes admitted to Lanzhou Second People's Hospital from June 2020 to June 2023 were selected as the observation group,and ninety healthy subjects were selected as the control group.Quantitative MRI was performed in both groups,and clinical data of the two groups were collected to compare pancreatic fat content,blood glucose,blood lipids and body mass index(BMI).The relationship between pancreatic fat content,blood glucose,blood lipid and BMI was analyzed.Results:The average fat fraction of pancreas,fat fraction of pancreatic head,body and tail,fasting blood glucose(FPG),low density lipoprotein cholesterol(LDL-C),triglyceride(TG),total cholesterol(TC)and body mass index(BMI)of observation group were higher than those of control group at all ages,the level of high density lipoprotein cholesterol(HDL-C)was lower than that of control group,and the difference was statistically significant(P<0.05);The mean fat fraction of pancreas,the fat fraction of pancreatic head,body and tail were positively correlated with FPG,LDL-C,TG,TC and BMI,and negatively correlated with HDL-C(P<0.05).Conclusion:The pancreatic fat fraction of type 2 diabetes patients was higher than that of healthy people,and the pancreatic fat content was closely related to the levels of blood glucose,blood lipids and BMI,indicating that pancreatic fat deposition was significantly associated with the development of type 2 diabetes and may be a risk factor for type 2 diabetes.
