摘要
目的:分析基层医院行关节镜辅助下经皮穿刺球囊成形术治疗SandersⅡA、ⅢAB型跟骨骨折的可行性及效果。方法:选取2021年4月—2022年3月中山市三角医院外一科收治的SandersⅡA、ⅢAB型跟骨骨折患者60例作为研究对象,采用随机表格法分为对照组和研究组,各30例。对照组采用切开撬拨复位内固定手术,研究组采用关节镜辅助下经皮穿刺球囊成形术治疗。比较两组术后足功能及跟骨解剖学指标情况、手术效果及安全性、术后关节功能恢复优良率。结果:术后,两组Maryland评分、跟骨长度、跟骨高度升高,跟骨宽度降低,且研究组变化幅度大于对照组,差异有统计学意义(P<0.05)。两组塌陷关节面再塌陷、术后感染及创伤关节炎发生率比较,差异无统计学意义(P>0.05);研究组骨折愈合时间短于对照组,差异有统计学意义(P<0.001)。研究组术后关节功能恢复优良率高于对照组,差异有统计学意义(P=0.039)。结论:基层医院行关节镜辅助下经皮穿刺球囊成形术治疗SandersⅡA、ⅢAB型跟骨骨折安全、有效,患者关节功能恢复情况好、骨折愈合时间短,有利于提高基层医院的治疗水平。
Objective:To analyze the feasibility and effect of percutaneous balloon plasty assisted by arthroscopy in the treatment of SandersⅡA andⅢAB calcaneal fractures in primary hospitals.Methods:Sixty patients with SanderⅡA andⅢAB calcaneal fracture admitted to the Department of First Surgery,Zhongshan Triangle Hospital from April 2021 to March 2022 were selected as the study objects,and were divided into control group and study group by random table method,with thirty cases in each group.The control group was treated with open pry reduction,and the study group was treated with arthroscopic assistance percutaneous balloon plasty.The postoperative foot function and calcaneus anatomic indexes,surgical effect and safety,and the excellent and good rate of postoperative joint function recovery were compared between the two groups.Results:After surgery,Maryland score,calcaneus length and calcaneus height increased,while calcaneus width decreased in both groups,and the change range in the study group was greater than that in the control group,with statistical significance(P<0.05).There was no significant difference in the rate of recollapse,postoperative infection and traumatic arthritis between the two groups(P>0.05);the fracture healing time of the study group was shorter than that of the control group,and the difference was statistically significant(P<0.001).The rate of joint function recovery in the study group was higher than that in the control group,and the difference was statistically significant(P=0.039).Conclusion:It is safe and effective to perform arthroscopy assisted percutaneous balloon plasty in the treatment of SandersⅡA andⅢAB calcaneal fractures in primary hospitals.The patients have good joint function recovery and short bone healing time,which is conducive to improving the treatment level of primary hospitals.
