摘要
目的:探讨经皮椎体成形术(PVP)治疗老年创伤性胸腰椎骨折的临床效果。方法:选取2019年1月—2023年5月宿迁市沭阳铭和医院收治的52例创伤性胸腰椎骨折患者作为研究对象,随机分为研究组和对照组,各26例。对照组采用保守治疗联合支具、辅助药物治疗,研究组采用PVP治疗。比较两组肿胀消退时间及住院时间、疼痛程度、脊柱功能、椎体前缘高度和后凸Cobb角。结果:研究组肿胀消退时间及住院时间均短于对照组,差异有统计学意义(P<0.001)。治疗前,两组视觉模拟评分法(VAS)、Oswestry功能障碍指数(ODI)评分比较,差异无统计学意义(P>0.05);治疗后,两组VAS、ODI评分均低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05)。治疗前后,两组椎体前缘高度、后凸Cobb角比较,差异无统计学意义(P>0.05);但治疗后,两组椎体前缘高度均高于治疗前,后凸Cobb角均小于治疗前,差异有统计学意义(P<0.001)。结论:PVP治疗老年创伤性胸腰椎骨折的效果显著,可有效减轻患者疼痛程度,改善脊柱功能,且恢复速度快。
Objective:To investigate the therapeutic effect of percutaneous vertebroplasty(PVP)in the treatment of traumatic thoracolumbar spine fracture in the elderly.Methods:52 patients with traumatic thoracolumbar vertebral fracture admitted to Shuyang Minghe Hospital in Suqian City from January 2019 to May 2023 were selected as the study subjects,and were randomly divided into the study group and the control group,each with 26 cases.The control group was treated with conservative treatment combined with Orthotics and auxiliary drugs,and the study group was treated with PVP.Swelling relieving time,hospitalization duration,pain level,spinal function,height of anterior vertebral margin and posterior convexity Cobb angle were compared between the two groups.Results:The swelling relieving time and hospitalization duration of the study group were shorter than that of the control group,and the difference was statistically significant(P<0.001).The visual analog scale(VAS)and Oswestry dysfunction index(ODI)scores of the two groups before operation were compared,and the difference was not statistically significant(P>0.05);the VAS and ODI scores of the two groups after treatment were lower than those before operation,and the study group was lower than the control group,and the difference was statistically significant(P<0.05).The height of the anterior margin of the vertebral body and the Cobb angle of the posterior convexity of the two groups were compared before and after the treatment,and the difference was not statistically significant(P>0.05);however,the height of the anterior margin of the vertebral body of both groups after treatment was higher than that before treatment,and the Cobb angle of the posterior convexity of the two groups was smaller than that before treatment,and the difference was statistically significant(P<0.001).Conclusion:The therapeutic effect of PVP in treating traumatic thoracolumbar fractures in the elderly is remarkable,which can effectively reduce the pain level of the patients,improve the spinal func
