摘要
目的 探究腹膜透析与血液透析在终末期糖尿病肾病治疗中的疗效。方法 选取2020年5月—2022年4月南京鼓楼医院集团宿迁医院收治的145例终末期糖尿病肾病(end-stage diabetic nephropathy, ESDN)患者为研究对象。按随机数表法将研究对象分为对照组(n=72)和研究组(n=73)。对照组行血液透析,研究组行腹膜透析。比较两组患者肾功能指标水平、临床指标水平、并发症发生率、生活质量。结果 治疗后,研究组各项肾功能指标水平均低于对照组,差异有统计学意义(P<0.001);研究组血白蛋白水平高于对照组,血清全段甲状旁腺激素、血磷水平低于对照组,差异有统计学意义(P<0.05);研究组并发症发生率(2.74%)低于对照组(12.50%),差异有统计学意义(χ^(2)=4.716,P=0.030);研究组生活质量评分高于对照组,差异有统计学意义(P<0.001)。结论 腹膜透析在ESDN患者中具有较高的临床疗效,有助于改善肾功能及临床指标水平,降低并发症发生风险,从而提高患者的生活质量。
Objective To explore the efficacy of peritoneal dialysis and hemodialysis in the treatment of end-stage diabetic nephropathy.Methods A total of 145 patients with end-stage diabetic nephropathy(ESDN)admitted to Suq-ian Hospital of Nanjing Drum Tower Hospital Group from May 2020 to April 2022 were selected as the research ob-jects.The subjects were divided into control group(n=72)and study group(n=73)according to the random number table method.The control group was treated with hemodialysis,and the study group was treated with peritoneal dialy-sis.The levels of renal function indexes,clinical indexes,incidence of complications and quality of life were compared between the two groups.Results After treatment,the levels of renal function indexes in the study group were lower than those in the control group,and the differences were statistically significant(P<0.001).The serum albumin level of the study group was higher than that of the control group,and the serum intact parathyroid hormone and blood phos-phorus levels were lower than those of the control group,and the differences were statistically significant(P<0.05).The incidence of complications in the study group(2.74%)was lower than that in the control group(12.50%),and the difference was statistically significant(χ^(2)=4.716,P=0.030).The quality of life score of the study group was higher than that of the control group,and the difference was statistically significant(P<0.001).Conclusion Peritoneal dialy-sis has a high clinical effect in ESDN patients,which helps to improve renal function and clinical indicators,reduce the risk of complications,and thus improve the quality of life of patients.
