功能性动作筛查在康复治疗师腰肌劳损中的应用研究 被引量:1

摘要 目的探索功能性动作筛查(FMS)在康复治疗师腰肌劳损评估中的应用,为劳损性疾病的预防提供科学依据。方法对18名康复治疗师进行腰肌劳损情况调查和FMS测评,以受试者工作曲线(ROC)作为康复治疗师产生腰肌劳损风险临界点的判断方法,采用卡方检验、点二列相关系数、多元logistic回归方程对FMS分数进行分析。结果以FMS>14.5为阴性组,FMS<14.5为阳性组,阴性组发生率为14.3%,阳性组发生率为63.6%(χ^(2)=7.901,P=0.019)。点二列相关系数研究结果显示,已知损伤和潜在损伤相关系数分别为-0.499、-0.808,P=0.035、0.000。多元logistic回归分析结果显示,仅主动直腿上抬测试可能是康复治疗师产生腰肌劳损的潜在保护性指标(P=0.035,OR=0.089),主动直腿上抬分数低的康复治疗师产生腰肌劳损是得分较高的康复治疗师的11(1/1.089)倍。结论功能性动作筛查能有效评估康复治疗师腰肌劳损,并起到积极预防作用。下肢柔韧性是康复治疗师产生腰肌劳损的主要原因,临床中应通过加强下肢柔韧性训练以达到减轻康复治疗师腰肌劳损发生的目的。 Objective To exploring the use of the Functional Movement Screen(FMS)in the assessment of lumbar muscle strain in physiotherapists to provide a scientific basis for the prevention of strain disorders.Methods A survey and FMS assessment of 18 physiotherapists in a rehabilitation medicine department for lumbar muscle strain,with Receiver Operating Characteristic Curve(ROC)as the method of judging the risk threshold of the LMS,to analyze FMS scores using Chi-square test,Permit-biserial correlation,and the multiple logistic regression.Results FMS scores in the negative and positive groups were>14.5,and<14.5,respectively;The incidence of LMS in the negative group was14.3%,and the incidence of LMS in the positive group was 63.6%(χ^(2)=7.901,P=0.019).The Permit-biserial correlation analysis showed that correlation coefficients of known injury and potential injury were-0.499 and-0.808,respectively(P=0.035,0.000).The Multiple Logistic regression results showed that only the size of the active straight leg raise scores might be the risk factor for physiotherapists(P=0.035,OR=0.089);and the risk occurrence of the low scores of active straight leg raise was 11 times(1/0.089)than the higher scores of LMS.Conclusion Functional movement screening can effectively assess lumbar muscle strain injury in physiotherapists and play a positive preventive role.Lower limb flexibility is the main cause of lumbar muscle strain injury in physiotherapists,and the clinical practice should be strengthened through lower limb flexibility training to reduce the occurrence of lumbar muscle strain injury in physiotherapists.
