摘要
目的探讨右美托咪定应用剂量对剖宫产产妇术后疼痛及康复的影响。方法选取2021年10月至2022年10月本院收治的150例接受剖宫产的初产妇,随机分为A组(0.375%罗哌卡因+生理盐水)、B组(0.375%罗哌卡因+0.5μg/kg右美托咪定)、C组(0.375%罗哌卡因+1.0μg/kg右美托咪定)。比较三组的VAS评分、初乳时间、泌乳素水平及不良反应发生情况。结果术后6 h、12 h,B组、C组VAS评分低于A组(P<0.05);B组、C组VAS评分比较差异无统计学意义(P>0.05)。A组初乳时间长于B组及C组,术后48 h泌乳素水平低于B组、C组(P<0.05);B组、C组初乳时间及术后48 h泌乳素水平比较差异无统计学意义(P>0.05)。A组恶心呕吐发生率高于B组、C组(P<0.05);C组心动过缓发生率高于A组、B组(P<0.05)。结论0.5μg/kg右美托咪定用于剖宫产产妇术后镇痛的效果最佳,利于产妇术后恢复,且不良反应发生率较低。
Objective To explore the impact of dexmedetomidine dosage on postoperative pain and recovery in puerperae with cesarean section.Methods 150 primipara with cesarean section in our hospital from October 2021 to October 2022 were selected and randomly divided into group A(0.375%ropivacaine+saline),group B(0.375%ropivacaine+0.5μg/kg dexmedetomidine)and group C(0.375%ropivacaine+1.0μg/kg dexmedetomidine).The VAS score,colostrum time,prolactin level and adverse reactions were compared among the three groups.Results 6 h and 12 h after operation,the VAS scores of group B and C were lower than those of group A(P<0.05);No statistical difference was found in the VAS score between group B and group C(P>0.05).The colostrum time of group A was longer than that of group B and group C,and the prolactin level at 48 h after operation was lower than that of group B and group C(P<0.05);No statistical difference was found in the colostrum time and prolactin level at 48 h after operation between group B and group C(P>0.05).The incidence of nausea and vomiting in group A was higher than that in groups B and C(P<0.05);The incidence of bradycardia in group C was higher than that in groups A and B(P<0.05).Conclusions 0.5μg/kg dexmedetomidine has best effect on postoperative analgesia in puerperae with cesarean section,which is conducive to postpartum recovery,with lower incidence of adverse reactions.
