摘要
目的探讨阴阳平衡针法联合靳三针治疗卒中后下肢痉挛性偏瘫的疗效。方法选取2020年3月—2022年3月三亚市中医院治疗的卒中后下肢痉挛性偏瘫患者116例,随机分为对照组(60例,靳三针疗法)和观察组(56例,阴阳平衡针法联合靳三针疗法),对比两组治疗后临床疗效、治疗前后Fugl-Meyer评测法(Fugl-Meyer assessment,FMA)评分、Berg平衡量表(Berg balance scale,BBS)评分和运动评估量表(motor assessment scale,MAS)、功能性步行量表(functional ambulation category,FAC)分级及血清γ-氨基丁酸(γ-aminobutyric acid,GABA)和谷氨酸(glutamate,Glu)水平变化。结果两组治疗后血清Glu和GABA水平、FAC分级较治疗前明显改善(P<0.05);且观察组改善更明显(P<0.05);两组治疗后FMA评分、BBS评分较治疗前均明显升高(P<0.05),MAS评分较治疗前均明显降低(P<0.05),且观察组改善更显著(P<0.05);治疗前观察组疗效明显较高(P<0.05);两组在心悸、胸闷等总发生率间差异无统计学意义(P>0.05)。结论采用阴阳平衡针法联合靳三针治疗卒中后下肢痉挛性偏瘫具有较好的疗效,可缓解下肢痉挛,改善下肢运动功能。
Objective To explore the effect of Yin-Yang balancing acupuncture combined with Jin′s three-needle therapy on spastic hemiplegia of lower limbs after stroke.Methods A total of 116 patients with spasmodic hemiplegia of lower extremity after stroke who were treated in the hospital from October 2019 to October 2021 were selected and randomly divided into control group(60 cases,Jin′s three-needle therapy)and observation group(56 cases,Yin-Yang balancing acupuncture combined with Jin′s three-needle therapy).The clinical efficacy,Fugl-Meyer assessment(FMA score),Berg balance scale(BBS)score,motor assessment scale(MAS),functional ambulation category(FAC)grade and changes of serumγ-aminobutyric acid(GABA)and glutamate(Glu)were compared between the two groups before and after treatment.Results The serum levels of Glu and GABA and FAC grade in two groups were significantly improved after treatment compared with those before treatment(P<0.05).The improvement was more obvious in the observation group(P<0.05).After treatment,FMA score and BBS score were significantly increased(P<0.05),MAS score was significantly decreased(P<0.05),and the improvement in the observation group was more significant(P<0.05).Before treatment,the curative effect of the observation group was significantly higher(P<0.05).There was no statistical difference in the total incidence of palpitation and chest tightness between the two groups(P>0.05).Conclusion The Yin-Yang balancing acupuncture combined with Jin′s three-needle therapy has a good effect on the treatment of spasmodic hemiplegia of lower limbs after stroke,which can relieve spasm of lower limbs and improve motor function of lower limbs.
