摘要
目的:探讨经阴道超声与性激素诊断多囊卵巢综合征(PCOS)合并不孕症的价值。方法:以78例PCOS合并不孕症患者为观察组,以同期78例健康女性为对照组。两组均接受阴道超声及性激素检测,分析超声及性激素诊断PCOS合并不孕症的价值。结果:观察组髓质面积、卵巢体积、卵巢总面积、髓质面积/卵巢面积高于对照组(P<0.05)。观察组EDV、PSV、RI高于对照组(P<0.05)。观察组LH、T、E2水平高于对照组,FSH水平低于对照组(P<0.05)。超声结果诊断PCOS合并不孕症的AUC、敏感度、特异度分别为0.865、89.74%、83.33%;血流参数诊断PCOS合并不孕症的AUC、敏感度、特异度分别为0.885、80.77%、89.74%;性激素诊断PCOS合并不孕症的AUC、敏感度、特异度分别为0.845、89.74%、79.49%;超声结果+血流参数诊断PCOS合并不孕症的AUC、敏感度、特异度分别为0.941、82.05%、93.59%。超声结果+血流参数诊断PCOS合并不孕症的效能高于性激素(P<0.05)。结论:阴道超声检查诊断PCOS合并不孕症的价值高于性激素。
Objective:To explore the value of transvaginal ultrasound and sex hormone in the diagnosis of polycystic ovary syndrome(PCOS)with infertility.Methods:78 patients with PCOS and infertility were selected as the observation group and 78 healthy women as the control group.Both groups received vaginal ultrasound and sex hormone testing to analyze the value of ultrasound and sex hormone in diagnosing PCOS with infertility.Results:The medullary area,ovarian volume,total ovarian area and medullary area/ovarian area of the observation group were higher than those of the control group(P<0.05).EDV,PSV and RI in the observation group were higher than those in the control group(P<0.05).The levels of LH,t and E2 in the observation group were higher than those in the control group,and the level of FSH was lower than that in the control group(P<0.05).The AUC,sensitivity and specificity of ultrasound in diagnosing PCOS with infertility were 0.865,89.74%and 83.33%respectively;The AUC,sensitivity and specificity of blood flow parameters in diagnosing PCOS with infertility were 0.885,80.77%and 89.74%respectively;The AUC,sensitivity and specificity of sex hormones in diagnosing PCOS with infertility were 0.845,89.74%and 79.49%respectively;The AUC,sensitivity and specificity of ultrasound and blood flow parameters in diagnosing PCOS with infertility were 0.941,82.05%and 93.59%,respectively.The efficacy of ultrasound and blood flow parameters in diagnosing PCOS with infertility was higher than that of sex hormone(P<0.05).
