摘要
目的 研究摄食训练联合吞咽凝胶干预对帕金森(PD)伴吞咽障碍患者的影响。方法 选取2021年1月—2023年1月医院收治的PD伴吞咽障碍患者110例为研究对象,按照组间基线资料均衡可比的原则分为观察组与对照组,各55例。在常规护理基础上,对照组实施吞咽凝胶干预,观察组在对照组基础上进行摄食训练干预,两组均干预2个月。采用中文版吞咽功能评估量表(GUSS)评价两组患者干预前后吞咽功能,采用用药满意度问卷(MSQ)、用药态度量表(DAI)评价患者的服药感受,采用Rosenbek渗透/误吸量表(PAS)评价患者的误吸风险,比较两组患者的误吸发生率,采用功能性经口摄食量表(FOIS)评价患者的经口摄食能力,采用吞咽障碍性异性生活质量量表(SWAL-QOL)评价患者的生活质量。结果 干预后,观察组患者PAS评分、误吸发生率低于对照组,组间比较差异均具有统计学意义(P<0.05);观察组患者GUSS评分、用药满意度、DAI评分、FOIS评级、SWAL-QOL评分高于对照组,组间比较差异均具有统计学意义(P<0.05)。结论 摄食训练联合吞咽凝胶干预PD伴吞咽障碍患者可提高其吞咽功能,改善患者服药感受,并降低误吸发生率。
Objective To study the effect offood intake training combined with swallowing gel intervention on patients with Parkinson's disease(PD) and dysphagia.Methods A total of 110 patients with PD and dysphagia admitted to the hospital from January 2021 to January 2023 were selected as the research subjects.According to the principle of balanced and comparable baseline data between groups,they were divided into an observation group and a control group,with 55 cases in each group.On the basis of routine care,the control group was given swallowing gel intervention,and the observation group was given feeding training intervention on the basis of the control group.Both groups were intervened for 2 months.Chinese version of the gugging swallowing screen(GUSS) was used to evaluate the swallowing function of the two groups of patients before and after intervention,the medication satisfaction questionnaire(MSQ) and drug attitude inventory(DAI)were used to evaluate the patients' medication experience,the Rosenbek penetration-aspiration scale(PAS) was used to evaluate the patients' risk of aspiration,the incidence of aspiration was compared between the two groups,and the functional oral intake scale(FOIS) was used to evaluate the patients' oral food intake ability,and swallowingrelated quality of life(SWAL-QOL) were used to evaluate patients' quality of life.Results After intervention,the PAS score and incidence of aspiration in the observation group were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05);the GUSS score,medication satisfaction,DAI score,FOIS rating and SWAL-QOL score of patients in the observation group were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Food intake training combined with swallowing gel intervention in patients with PD and dysphagia can improve their swallowing function,improve patients' medication experience,and reduce the incidence of aspiration.
