摘要
目的 调查北京市朝阳区老年HIV/AIDS患者新型冠状病毒疫苗(以下简称“新冠疫苗”)的接种情况,并探讨其可能的影响因素,为进一步推进老年HIV/AIDS患者疫苗接种工作提供参考依据。方法 通过历史随访信息和自制的调查问卷收集160名老年HIV/AIDS患者的相关信息,采用χ^(2)检验、Fisher确切概率法和logistic回归模型分析HIV感染者新冠疫苗接种情况的差异及其影响因素。结果 本研究中老年HIV/AIDS患者新冠疫苗接种率为76.3%。单因素分析发现,基础疾病患病情况、对新冠疫苗安全性和有效性的态度、医生建议和CD4~+T细胞数量与新冠疫苗接种有关。logistic回归分析显示,患有基础疾病(OR=0.201,95%CI:0.057~0.703)、CD4~+T细胞数量<200个/μL(OR=0.059,95%CI:0.004~0.960)和临床医生的消极建议(OR=0.044,95%CI:0.005~0.364)是老年HIV/AIDS患者接种率提高的不利因素,而对新冠疫苗安全性的认可(OR=6.464,95%CI:1.702~24.547)和临床医生的积极建议(OR=4.114,95%CI:1.002~16.895)则是接种率提高的促进因素。结论 老年HIV/AIDS患者新冠疫苗接种率相对较低,应该加强针对该人群的临床试验研究,及时披露相关信息、开展针对性的健康教育,并鼓励该群体寻求专业的医疗帮助,以增强其接种新冠疫苗的意愿和信心。
Objective To investigate the vaccination status of COVID-19 vaccine among elderly HIV/AIDS patients in Chaoyang district of Beijing,and to analysis the possible influencing factors,so as to provide reference evidence for further promoting the vaccination of these people.Methods The information of 160 elderly HIV/AIDS patients were collected through the follow-up database and self-made questionnaire.Chi-square test,Fisher's exact test and logistic regression model were used for data statistical analysis.Results In this study,the coverage of COVID-19 vaccine was 76.3%.Univariate analysis showed that the chronic diseases,attitudes towards the safety and effectiveness of COVID-19 vaccine,doctors'recommendations,and CD4*T cell count were associated with COVID-19 vaccination.Logistic regression analysis showed that presence of chronic disease conditions(0R=0.201,95%Cl:0.057-0.703),lower than 200/μL CD4*T cell counts(0R=0.059,95%CI:0.004-0.960)and the doctors'negative suggestions(0R=0.044,95%CI:0.005-0.364)were negative factors to improve the coverage of elderly HIV/AIDS patients,while the recognition of the safety of COVID-19 vaccine(OR=6.464,95%CI:1.702-24.547)and doctors positive suggestions(0R=4.114,95%Cl:1.002-16.895)were positive factors for the vaccination.Conclusions The coverage of COVID-19 vaccine in elderly HIV/AIDS patients was relatively low and needed to be improved.More clinical trials should be conducted in this population.It is necessary to disclose relevant information timely,carry out targeted health education,and encourage these population to seek professional medical help,so as to enhance their willingness and confidence to be receive the vaccine of COVID-19.
