摘要
目的 探讨血清分形趋化因子(FKN)、β2微球蛋白(β2-MG)、肿瘤坏死因子α刺激基因6(TSG-6)在扩张型心肌病中的表达及与心室重构的关系。方法 选择2021年1月至2023年8月于湖北省中西医结合医院收治的扩张型心肌病患者73例设为病例组,另选取同期我院体检健康65例设为对照组。病例组中根据NYHA心功能分级分为Ⅱ级22例、Ⅲ级25例、Ⅳ级26例。检测并分析两组患者的血清FKN、β2-MG及TSG-6水平变化,同时测量并分析其与左房内径(LAD)、左室舒张末期内径(LVEDd)、右心室内径(RVD)、左室短轴缩短率(LVFS)、左室射血分数(LVEF)之间的关系。结果 病例组患者血清FKN、β2-MG及TSG-6水平显著高于对照组,差异有统计学意义(P<0.05);Ⅱ级组患者血清FKN、β2-MG及TSG-6水平显著低于Ⅲ级组、Ⅳ级组患者,Ⅲ级组患者血清FKN、β2-MG及TSG-6水平显著低于Ⅳ级组患者(P<0.05);病例组患者LAD、LVEDd、RVD水平显著高于对照组,LVFS、LVEF水平显著低于对照组,差异有统计学意义(P<0.05);相关性分析显示,扩张型心肌病患者血清FKN、β2-MG及TSG-6均与LAD、LVEDd、RVD之间呈正相关,与LVFS、LVEF之间呈负相关(P<0.05)。结论 扩张型心肌病患者FKN、β2-MG及TSG-6水平表达异常,且与心室重构之间存在相关性,上述指标可能协同参与了扩张型心肌病的发生及发展。
Objective To investigate the expressions of serum fractalkine(FKN),β2-microglobulin(β2-MG) and tumor necrosis factor α stimulating gene 6(TSG-6) in dilated cardiomyopathy(DCM) and relationship among them and ventricular remodeling.Methods DCM patients(n=73) were chosen into case group and healthy controls(n=65) were chosen into control group from Hospital of Integrated Traditional and Western Medicine of Wuhan City from Jan.2021 to Aug 2023.According to NYHA classification of heart function,the case group were divided into grade Ⅱ subgroup(n=22),grade Ⅲ subgroup(n=25) and grade Ⅳ subgroup(n=26).The level changes of serum FKN,β2-MG and TSG-6 were detected and analyzed.The relationship among left atrial diameter(LAD),left ventricular end-diastolic inner diameter(LVEDd),right ventricular diameter(RVD),left ventricular fraction shortening(LVFS) and left ventricular ejection fraction(LVEF) and levels of serum FKN,β2-MG and TSG-6was analyzed.Results The levels of serum FKN,β2-MG and TSG-6 were significantly higher in case group than those in control group(P<0.05).The levels of serum FKN,β2-MG and TSG-6 were significantly lower in grade Ⅱ subgroup than those in grade Ⅲ subgroup and grade Ⅳ subgroup,and were significantly lower in grade Ⅲ subgroup than those in grade Ⅳ subgroup(P<0.05).The levels of LAD,LVEDd and RVD were significantly higher,and levels of LVFS and LVEF were significantly lower in case group than those in control group(P<0.05).The results of correlation analysis showed that levels of serum FKN,β2-MG and TSG-6 were positively correlated to LAD,LVEDd and RVD,and negatively correlated to LVFS and LVEF(P<0.05).Conclusion The expressions of serum FKN,β2-MG and TSG-6 are abnormal,and they are correlated to ventricular remodeling,which may synergistically take part in the occurrence and development of DCM.
