摘要
目的 探讨在老年重症心力衰竭患者急诊治疗中,采用内科综合治疗方案的价值。方法 选取2021年2月—2023年2月于北京市平谷区医院确诊的60例重症心力衰竭老年患者的临床资料进行回顾性研究,以治疗方案不同分为两组,各30例。对照组实施内科常规治疗,观察组实施内科综合治疗。比较两组治疗后临床疗效、不良反应发生率、心功能、血流动力学指标。结果 观察组临床疗效96.67%高于对照组,差异有统计学意义(χ~2=4.043,P=0.044);观察组不良反应发生率与对照组相比,差异无统计学意义(P>0.05);观察组左心室射血分数、左心室舒张末期容积高于对照组,左室舒张末期内径、左室收缩末期内径低于对照组,差异有统计学意义(P<0.05);观察组体循环阻力低于对照组,一氧化氮高于对照组,差异有统计学意义(P<0.05)。结论 在老年重症心力衰竭治疗中选择内科综合治疗方案可提高病情控制效果,亦可改善其心功能及血流动力学,值得借鉴与实施。
Objective To explore the value of comprehensive medical treatment in emergency treatment of senile patients with severe heart failure.Methods The clinical data of 60 elderly patients with severe heart failure diagnosed in Pinggu District Hospital of Beijing from February 2021 to February 2023 were selected for retrospectively study.According to different treatment plans,they were divided into two groups,with 30 cases in each group.The control group was given routine medical treatment,and the observation group was given comprehensive medical treatment.SPSS.Compared the clinical efficacy,the incidence of adverse reactions after treatment,cardiac function and hemodynamic indexes.Results The clinical efficacy of the observation group 96.67% was higher than that of the control group,the difference was statistically significant(χ~2=4.043,P=0.044).There was no statistically significant difference in the incidence of adverse reactions between the observation group and the control group(P>0.05).The left ventricular ejection fraction and left ventricular end-diastolic volume in the observation group were higher than those in the control group,and the left ventricular end-diastolic diameter and left ventricular end-systolic diameter were lower than those in the control group,and the differences were statistically significant(P<0.05).The systemic circulatory resistance of the observation group was lower than that of the control group,and the nitric oxide was higher than that of the control group,and the differences were statistically significant(P<0.05).Conclusion In the treatment of senile severe heart failure,the selection of comprehensive medical treatment can improve the disease control effect,cardiac function and hemodynamics,which is worthy of reference and implementation.
