摘要
痛风是常见的代谢性疾病之一,痛风石的形成严重影响着生活质量,早期干预及预防是避免痛风发生发展的关键,2015年美国风湿病协会/欧洲抗风湿联盟联合推荐将双能CT纳入评分标准,以双能CT为代表的现代技术的应用能直观地显示痛风石的数目、位置、大小和形态,从痛风石分布的规律可以发现足太阴、足阳明、足厥阴经及其相连属的经筋、皮部、五腧穴、阿是穴等在痛风的病理过程中发挥着关键的作用,包括痛风急性发作期的阳明热毒、厥阴火盛、太阴湿浊;慢性关节炎期的脾虚湿盛、肝肾亏虚、浊毒瘀阻等是目前临床医家利用针刺、膏摩、针刀、微创等外治法治疗痛风循经选穴的依据,也指导着内科的辨证治疗,加深了我们对痛风中医病因、病机及治疗的认识。
Gout is one of the common metabolic diseases.The formation of tophus seriously affects the quality of life.Early intervention and prevention are the key to avoid the occurrence and development of gouty arthritis.The application of modern technology represented by dual-energy CT can visually display the number,location,size and shape of gouty tophus.From the distribution rule of tophus,it can be found that spleen meridian of foot-Taiyin,stomach meridian of foot-Yangming,liver meridian of foot-Jueyin and their muscle regions,cutaneous regions,five Shu points and Ashi points play key roles in the pathological process of gout.Including the acute gouty arthritis in Yangming heat toxin,Jueyin fire,dampness stranding the spleen;In the chronic arthritis stage,spleen deficiency and dampness,liver and kidney deficiency,turbidity,poison and blood stasis are the basis of acupoint selection along meridians by clinical doctors in the treatment of gout with external treatment such as acupuncture,plaster rubbing,acupotomology and minimally invasive therapy.It also guides the syndrome differentiation treatment of internal medicine,and deepening our understanding of the etiology,pathogenesis and treatment of gout in traditional Chinese medicine.
