摘要
目的探讨基于“脾为生痰之源”以健脾化痰汤治疗慢性阻塞性肺疾病(COPD)的疗效。方法选取2020年12月—2022年5月三明市中西医结合医院呼吸内科收治的60例COPD(痰浊壅肺型)的患者,采用随机数字表法分为试验组和对照组各30例。对照组予以常规西药治疗,试验组加用健脾化痰汤,两组均治疗1周,比较两组临床症状评分、肺功能、炎性反应指标、中医证候积分、临床疗效。结果治疗后,试验组总有效率高于对照组(P<0.05);两组CASA-Q评分均提高(P<0.05),试验组高于对照组(P<0.05);两组FVC、FEV_(1)、FEV_(1)/FCV均提高(P<0.05),试验组均高于对照组(P<0.05);两组CRP、IL-6均降低(P<0.05),试验组均低于对照组(P<0.05);两组中医证候积分均降低(P<0.05),试验组低于对照组(P<0.05)。结论基于“脾为生痰之源”以健脾化痰汤联合常规西药治疗COPD可提高临床疗效,降低炎性反应,改善临床症状和中医证候,值得推广。
Objective Based on the"source of the spleen for the sputum"to explore the effect of Jianpi Huatan Decoction(健脾化痰汤)for the treatment of chronic obstructive pulmonary disease.Methods 60 patients with COPD(Tanzhou Yongfei)admitted to the Department of Respiratory Medicine of Sanming Hospital of Integrated Traditional Chinese and Western Medicine from December 2020 to May 2022,and divided into 30 cases of test group and control group by random number table.The control group was treated with western medicine,the experimental group was treated with Jianpi Huatan Decoction.Both groups were treated for one week.Clinical symptom scores,lung function,inflammatory response indicators,traditional Chinese medicine syndrome scores,and clinical efficacy were compared between the two groups before and after treatment.Results After treatment,the total effective rate of the experimental group was higher(P<0.05).The CASA-Q scores,FVC,FEV_(1),FEV_(1)/FCV of both groups improved(P<0.05),and the experimental group were higher(P<0.05).CRP,IL-6 levels and traditional Chinese medicine syndrome points decreased in both groups(P<0.05),and the experimental group were lower(P<0.05).Conclusion Based on the"source of the spleen for the sputum",the Jianpi Huatan Decoction combined with conventional western medicine can improve the clinical effect,reduce inflammatory response,improve clinical symptoms and traditional Chinese medicine symptoms,which is worth promoting.
