摘要
目的探究动态心电图检查阵发性心房颤动(paroxysmal atrial fibrillation,PAF)患者特征及心率变异性(heart rate variablity,HRV)。方法选取2022年6月—2023年6月潮州市人民医院接受检查的120例PAF患者纳入观察组,另择同期检查的100例无房颤患者纳入对照组。两组患者均进行24 h动态心电图检查,比较PAF患者发作前、发作后1 h的总心搏数和平均心率,比较两组患者心率变异性时域指标。结果PAF患者发作后1 h的总心搏数和平均心率较发作前1 h显著升高,差异有统计学意义(P<0.05)。观察组患者全部窦性心搏RR间期标准差,RR间期平均值标准差,相邻RR间期差值的均方根,相邻NN之差>50ms的个数占总窦性心搏个数的百分比指标的水平显著高于对照组,差异有统计学意义(P<0.05);观察组患者低电压(low frequency,LF)(1261.69±1349.69 ms2/H2)、高电压(high frequency,HF)(3372.88±3845.02 ms2/H2)、LF/HF(6.58±2.72)指标的水平显著高于对照组,差异有统计学意义(t=7.820、5.535、10.371,P<0.05)。结论动态心电图检查能有效显示PAF患者特征,PAF患者发作后1 h的总心搏数和平均心率较发作前1 h显著升高,PAF患者心率变异性较高,且并发自主神经功能紊乱。
Objective To explore the characteristics and heart rate variability(HRV)of patients with paroxysmal atrial fibrillation(PAF)by holter electrocardiogram.Methods A total of 120 patients with PAF who were examined in Chaozhou People's Hospital from June 2022 to June 2023 were included in the observation group,and another 100 patients without atrial fibrillation who were examined during the same period were included in the control group.All patients underwent 24-hour dynamic electrocardiogram examination.The total stroke volume and mean heart rate were compared before and 1 hour after the onset of PAF,and the time domain index of heart rate variability was compared between the two groups.Results Patients with PAF after 1 h of the total number of cardiac and average heart rate before the onset of a significant rise in 1 h,the differences were statistically significant(P<0.05).The levels of standard diviation of NN intervals,standard diviation average of NN intervals,square root of the mean squared differences of successive RR intervals,percentage of differences exceeding 50 ms between adjacent normal number ofintervals in the observation group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).The levels of low frequency(1261.69±1349.69 ms2/H2),high frequency(3372.88±3845.02 ms2/H2)and LF/HF(6.58±2.72)in the observation group were significantly higher than those in the control group,and the differences were statistically significant(t=7.820,5.535,10.371,P<0.05).Conclusion Holter examination can effectively display the characteristics of PAF patients.The total heart beat number and average heart rate of PAF patients 1 h after the onset of PAF are significantly higher than that of the patients 1 h before the onset of PAF.The heart rate variability of PAF patients is higher,and the autonomic nervous dysfunction is complicated.
