摘要
目的:评价粤港澳大湾区背景下广东省中医类医院卫生资源配置公平性,为广东中医药强省、大湾区中医药高地建设的政策制定提供参考。方法:运用泰尔指数和集中指数方法描述2011—2020年广东省中医类医院卫生资源配置现状及公平性情况。结果:2011—2020年间,广东省中医类医院卫生资源按人口配置的总泰尔指数在0.099~0.261;按地理配置的总泰尔指数为0.635~1.000,床位数、卫生技术人员、执业(助理)医师、注册护士的区域间泰尔指数贡献率为57%~66%,均大于其区域内泰尔指数贡献率(34%~43%);2011—2020年,广东省中医类医院床位数、卫生技术人员、执业(助理)医师、注册护士数的集中指数分别为0.000~0.162、0.064~0.186、0.088~0.175、0.065~0.222,均呈逐年下降趋势,机构数集中指数为-0.125~-0.005。结论:2011—2020年港澳及广东省中医类医院卫生资源总量显著增加,符合粤港澳大湾区中医药高地建设的发展趋势;广东省按人口配置的中医类医院卫生资源公平性远小于按地理配置的,而地理公平性差异主要是来自区域间的差距。
Objective:To evaluate the equity of health resource allocation for TCM hospitals in Guangdong Province in the context of Guangdong-Hong Kong-Macao Greater Bay Area(the Greater Bay Area)and to provide reference for the policy formulation of Guangdong's TCM powerhouse and the Greater Bay Area's TCM highland.Methods:Theil index and concentrated index method were used to describe the status quo and equity of health resource allocation in TCM hospitals in Guangdong Province from 2011 to 2020.Results:From 2011 to 2020,the total Theil index of health resources allocation per population in TCM hospitals in Guangdong Province ranged from 0.099 to 0.261.The variation range of the total Theil index by geographical allocation was 0.635~1.000,and the contribution rate of the inter-regional Theil index of the number of sick beds,health technicians,practicing(assistant)physicians and registered nurses was 57%~66%,which was greater than that of the intra-regional Theil index(34%~43%).From 2011 to 2020,the concentration index of the number of beds,health technicians,practicing(assistant)physicians,and registered nurses in TCM hospitals in Guangdong Province showed a decreasing trend year by year in the ranges of 0.000~0.162,0.064~0.186,0.088~0.175,and 0.065~0.222,respectively.The concentration index of the number of mechanisms ranges from−0.125 to−0.005.Conclusion:From 2011 to 2020,the total health resources of TCM hospitals in Hong Kong,Macao and Guangdong Province increased significantly,which is in line with the development trend of the construction of TCM highland in the Guangdong-Hong Kong-Macao Greater Bay Area;In Guangdong Province,the equity of health resources of TCM hospitals allocated according to population was much smaller than that allocated according to geography,and the difference of geographical equity mainly come from the gap between regions.
