摘要
目的探讨舒芬太尼麻醉复合地佐辛方案在老年腹腔镜胆囊切除术(laparoscopic cholecystectomy,LC)患者中的应用价值。方法选取80例镇江市丹徒区人民医院2018年7月—2023年5月收治的老年LC患者,按随机数表法分为对照组和研究组,各40例。两组均行气管插管全身麻醉,对照组予以舒芬太尼麻醉,观察组予以舒芬太尼复合地佐辛麻醉。比较两组苏醒质量指标、术后疼痛评分、血流动力学变化、炎症反应及不良反应。结果观察组睁眼时间、呼吸恢复时间及拔管时间为(5.63±1.12)、(4.36±0.54)、(6.74±1.15)min,短于对照组的(7.63±1.24)、(6.47±1.15)、(8.93±1.26)min,差异有统计学意义(t=7.570、10.504、8.119,P<0.05);观察组术后6 h、12 h及24 h时视觉模拟评分法评分低于对照组,差异有统计学意义(P<0.05);观察组T2时心率、舒张压、收缩压高于对照组,差异有统计学意义(P<0.05);观察组术后超敏C反应蛋白、肿瘤坏死因子-α及白细胞介素-6水平低于对照组,不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论舒芬太尼复合地佐辛在老年LC患者中应用价值更高,可减轻术后疼痛,缩短睁眼及呼吸恢复时间,降低术后炎症反应,安全可靠。
Objective To explore the application value of sufentanil anesthesia combined with dezocine in elderly patients with laparoscopic cholecystectomy(LC).Methods A total of 80 elderly patients with LC admitted to Dantu District People's Hospital from July 2018 to May 2023 were selected and divided into control group and observation group with forty cases each according to random number table method.Both groups were given tracheal intubation general anesthesia,the control group was given sufentanil anesthesia,the observation group was given sufentanil combined with dezocine anesthesia.The indexes of recovery quality,postoperative pain score,hemodynamic changes,inflammatory response and adverse reactions were compared between the two groups.Results The time of eye opening,respiratory recovery and extubation in the observation group were(5.63±1.12)min,(4.36±0.54)min,(6.74±1.15)min,which were shorter than those of the control group(7.63±1.24)min,(6.47±1.15)min,(8.93±1.26)min,and the difference was statistically significant(t=7.570,10.504,8.119,P<0.05).The Visual Analogue Scale scores of the observation group at 6-hour,12-hour and 24-hour were lower than those of the control group,and the difference was statistically significant(P<0.05).The heart rate,diastolic blood pressure and systolic blood pressure in the observation group were higher than those in the control group at T2,and the differences were statistically significant(P<0.05).The levels of hypersensitive C-reactive protein,tumor necrosis factor-αand interleukin-6 in the observation group were lower than those in the control group,and the incidence of complications was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion Sufentanil combined with dezocine has higher application value in elderly patients with LC,which can reduce postoperative pain,shorten the recovery time of eye opening and breathing,reduce postoperative inflammation,and is safe and reliable.
